Patient Grid

The patient grid displays the patient demographic details, financial and next appointment details. When you run the NextGen Care® Population Management Hub report, patient information appears in the patient grid. The patient grid displays the patient demographic details, patient's risk level (color-coded), patient portal enrollment status, financial information, and the next appointment details. Note: If multiple appointments are scheduled for a patient on the same day, the next immediate appointment of that day appears in the Next Appointment field when you run the report. When reports are filtered based on appointment schedule in the Appointment Book tab, the patient grid displays additional columns. Note: The following columns appear in the patient grid when they are enabled on the Grid + Data tab on the User Settings window. You can filter the patients based on the NextGen® Enterprise PM schedule data to support care management work-flows such as care team huddles and pre-visit planning. Note: The Appointment Book tab is not available in NextGen Care® Population Management Hub, if the View access rights for Modules > Medical Records > Patient Appointment is turned off in the System Administrator. You can either use one filter or all the filters on the Appointment Book tab to filter the patients. If you do not select any filter such as locations, date, time, resources, and events, then the system responds as if all the options for the filter were selected. For example, if you do not select locations, then all the locations of that practice appears, relative to other filters selected. Configure User Settings

On the User Settings tab, you can customize the display for Cohorts, patient grid, Chart, and Care Opportunities. You can select the following: The report type to be displayed on the tab. The financial information columns for the patient grid. The templates that appear in the Chart list. The care opportunity layout for the Care Opportunities tab. Open NextGen Care® Population Management Hub. Select the settings icon in the lower-right corner. The Settings window opens. To configure the reports to be displayed on the Cohorts tab, select Reports on User Settings and select the desired reports. To configure the patient details to be displayed in the patient grid, do the following: Select Grid + Data. In the Cohorts section, select Limit Snapshots to last to limit snapshots to a specified number of days and enter the number of days to which the snapshots must be limited. Configure the Global Settings

You can set up the dashboard template, referral template, and document format in the Global Settings tab. From NextGen Care® Population Management Hub, select settings icon at the bottom-right corner, and then select Global Settings. The Global Settings window opens. Under Dashboard, enter the dashboard template name. Under Referral, referral template name appears. if required, you can modify the referral template name. Under Document, select the document format from the Format list. Select OK. The dashboard template, referral template, and document format are set up. Enter Population Health Global Settings

The first time you set up Population Health Sync reporting in NextGen® Enterprise EHR, you must enter global system settings for it in Enterprise Preferences. These settings apply to all enterprises in NextGen® Enterprise EHR who use Population Health Sync. The settings are retained and do not need to be entered again. To obtain the global settings values, contact your Population Health Sync support team. The information may be sent to you by email or instant messaging. Under Population Health Settings > Global Settings, enter the values. To save the global settings before you enter the credential settings, select OK.
