Open the Assessments Tab

There are several options for accessing the Add or Update Assessments template: On the SOAP template, in the Assessment / Plan panel, select Add/Update. On the Intake or SOAP templates: Select the My Plan link to open the template set. My Plan Select the Assessments tab. Access the Add or Update Assessment Template

Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click Add/Update. The Add or Update Assessment template opens. Add or Update Assessment Template

The Assessments link in the My Plan template enables you to open the Add or Update Assessment template. The Add or Update Assessment template enables you to add and update assessments. Add or Update Assessment Grids in Add or Update Assessment Template Open the Assessments Tab Patient's Treatment Plan The treatment plan for the patient can be documented on the My Plan or My Custom Plan template. Patient's Lab Orders You can place lab orders from the Labs tab using the Lab Master template or the Orders console. The Process and Submit Lab/Radiology Orders template shares information with the Lab Master template and the Orders console. Place an Order for Diagnostic Studies You can place a diagnostic study order with Bypass Order Module Processing enabled. Delete an Existing Assessment

CAUTION: Removing an item from the Today's Assessment grid does not resolve the diagnosis. The Add or Update Assessments template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates as the working assessment. If you remove one of multiple available assessments on the Add or Update Assessments template, you must select a new working assessment that carries over to the other templates that share the same working assessment. For more information, see Shared Working Assessment On the Assessments tab , in the Today's Assessment grid, select the row to be deleted. Click Remove. Click Save & Close. Bypassing the Add/Update Button When Entering Assessments

Practice Preferences Selecting Add Assessments on 1 click enables you to include the selected diagnosis description and code to the Today's Assessment grid without having to click Add/Update: If the practice setting default is set to Add Assessments on 1 click, upon opening the template, the check box is automatically selected by default and the one-click functionality is active. If the practice setting is not turned on, the check box is not selected upon opening the Add or Update Assessment template. After the Add or Update Assessment template opens, any changes to the selection of the Add Assessments on 1 click check box remains until it is either manually changed again or a new encounter is opened. Document an Assessment

To add or edit a diagnosis on the Add or Update Assessment template, use one of the following sections: Billing Diagnosis History Clinical Problems My Favorites The new or revised diagnosis appears in the list of diagnoses in the Today's Assessment grid. If a working assessment does not exist, the diagnosis information appears in the ICD description and ICD Code fields. If your practice required a status when you selected the diagnosis, it appears in the Status field. If a working assessment exists, it remains selected in the Today's Assessment grid, and the ICD description, ICD code, and status for the working assessment are retained. To change the working assessment, select the diagnosis being addressed for this encounter in the Today's Assessment grid. The selected diagnosis becomes the new working assessment that carries forward to the other templates that share the assessment. To change the ICD code, click the ICD Code field and search for and select the correct code for the diagn
