Remove an Assessment from Add or Update Assessment Template

If you remove an assessment from the Add or Update Assessment template, you must add an assessment to carry over to other templates that share the same working assessment. Open NextGen® Enterprise EHR. On the Add or Update Assessment template, select the Assessments tab. In the Today's Assessment grid, select an assessment. Select Remove. Select Save & Close. Sort Assessment Order

For billing or documentation purposes, it may be necessary to modify the order in which the assessments are displayed on your documentation. From the Assessment/Diagnosis panel, select Sort. The Reorder Assessments window opens. Reorder Assessments window From the Existing Sort Order grid, select the assessment to display first in the list. The assessment displays as #1 in the New Sort Order grid. From the Existing Sort Order grid, select the assessment to display next in the list. The assessment displays as #2 in the New Sort Order grid. Repeat step 3 until all assessments on the encounter display correctly in the list. Reorder Assessments window To move the remaining assessments without changing the order to the New Sort Order grid, select Move Remaining. Reorder Assessments

Assessments can be sorted into a preferred order using the Reorder Assessments template. Click the Sort button to open the Reorder Assessments template. reorder assessments template Start with the assessment that you want to be displayed in the list first. Click the assessments in the Existing Sort Order grid in the order you want them to appear in the New Sort Order grid one at a time. Entering and Modifying Assessments

The Add or Update Assessment template is found on the Assessments tab, which addresses the needs of both ICD-10 coding and 5010 standards. The entered assessments display in a grid format to enable the submission of 12 diagnoses on a claim. The grid for Today’s Assessments on the Assessments tab contains the following elements: Today's Assessments First column (#) indicates the priority; important when more than 12 row items exist as only the first 12 can be submitted on the claim Second column holds the diagnosis description, diagnosis code, and if entered, the Status, Side, and Site Third column is a separated concatenation of the impression and differential diagnoses Note: If an HCC column appears in the grid, accessing the Problems module is required. Using the Assessments Tab

The Assessments tab in the My Plan template set houses the Add or Update Assessment template. The grids on the Assessments tab can be filtered. For example, in the Billing Diagnosis History section, selecting the Show Chronic only check box displays only those diagnoses that are not acute. Selecting the option to Add Assessments on 1 click speeds up the documentation process by including the diagnosis description and code for Today's Assessment in the grid and Problems module without having to click Add/Update. The Medications or Immunizations links can be used at any time during the encounter to access the associated module. Place Orders from the My Plan Template

You can place orders for the patient using the My Plan template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link. The Care Guidelines grid view template opens. Expand the Recent Orders panel and select Add. The My Plan template opens. My Plan The My Plan template displays order items in the set appropriate for the selected assessment’s diagnosis category. To use a different template in the orders set, select the tab corresponding to the template to use (for example, Labs or Diagnostics). To add or change a diagnosis in the Today's Assessments grid, select Add or Edit. To change the current working assessment, select the diagnosis addressed for this encounter in the Today's Assessments grid. The selected diagnosis appears in the Selected Dx field.
