Add Diagnostic Details

In the Assessment/Diagnosis panel, confirm the assessment selected displays in the Description details and ICD Code field. If not, select the assessment in the grid to fill in the details. Add Status, Severity, Differential Dx, and Impression. To enter specifiers in the Specifiers field, do one of the following: Select in the field and enter free text. Select the My Phrases link and select a phrase from the list. If applicable, select Present on Admission and then select a description from the list. To indicate the diagnosis is chronic, add the diagnosis to the client’s clinical problems list, and/or add the assessment to my tracked problems, select the respective checkbox. Select Add To Grid. The information displays in the grid. To add psychosocial stressors, select the Psychosocial Stressors link. To add referrals, select the Referrals link. To reorder the diagnoses, select Sort. Reorder Assessments

Assessments can be sorted into a preferred order using the Reorder Assessments template. Click the Sort button to open the Reorder Assessments template. reorder assessments template Start with the assessment that you want to be displayed in the list first. Click the assessments in the Existing Sort Order grid in the order you want them to appear in the New Sort Order grid one at a time. Change the Order of Assessments in the Grid

Because only the first 12 line items are available for billing, providers may want to ensure that certain assessments are included in the top 12 rows for the list. Open NextGen® Enterprise EHR. On the Assessments tab, select Sort to open the Reorder Assessments template. Reorder Assessments In the Existing Sort Order grid, select the row that you would like to be the first in the list. That selection becomes number 1 in the New Sort Order grid. Continue to select the available assessment rows until you have them in the desired order. If you select a row in error, select Undo Last. Add a New Assessment Using the Diagnosis Search

You can search for an assessment using the diagnosis search window. Click the Diagnosis Lookup link. The diagnosis search window opens. diagnosis lookup link Enter the first few letters of the desired assessment in the Search field. Click Search. The results grid refreshes to display a list of assessments that include the searched for item. Highlight a row in the results grid. Click Select. The selected assessment appears in the Today's Assessments grid. You can open the Add or Update Assessment template using the Assessments link in the My Plan template. The Add or Update Assessment template allows you to add and update assessments.
