Access the Add or Update Assessment Template

Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click Add/Update. The Add or Update Assessment template opens. Add or Update Assessment Template

The Assessments link in the My Plan template enables you to open the Add or Update Assessment template. The Add or Update Assessment template enables you to add and update assessments. Add or Update Assessment Grids in Add or Update Assessment Template Open the Assessments Tab Patient's Treatment Plan The treatment plan for the patient can be documented on the My Plan or My Custom Plan template. Patient's Lab Orders You can place lab orders from the Labs tab using the Lab Master template or the Orders console. The Process and Submit Lab/Radiology Orders template shares information with the Lab Master template and the Orders console. Using the Assessments Tab

The Assessments tab in the My Plan template set houses the Add or Update Assessment template. The grids on the Assessments tab can be filtered. For example, in the Billing Diagnosis History section, selecting the Show Chronic only check box displays only those diagnoses that are not acute. Selecting the option to Add Assessments on 1 click speeds up the documentation process by including the diagnosis description and code for Today's Assessment in the grid and Problems module without having to click Add/Update. The Medications or Immunizations links can be used at any time during the encounter to access the associated module. Document an Assessment

To add or edit a diagnosis on the Add or Update Assessment template, use one of the following sections: Billing Diagnosis History Clinical Problems My Favorites The new or revised diagnosis appears in the list of diagnoses in the Today's Assessment grid. If a working assessment does not exist, the diagnosis information appears in the ICD description and ICD Code fields. If your practice required a status when you selected the diagnosis, it appears in the Status field. If a working assessment exists, it remains selected in the Today's Assessment grid, and the ICD description, ICD code, and status for the working assessment are retained. To change the working assessment, select the diagnosis being addressed for this encounter in the Today's Assessment grid. The selected diagnosis becomes the new working assessment that carries forward to the other templates that share the assessment. Remove an Assessment from Add or Update Assessment Template

If you remove an assessment from the Add or Update Assessment template, you must add an assessment to carry over to other templates that share the same working assessment. Open NextGen® Enterprise EHR. On the Add or Update Assessment template, select the Assessments tab. In the Today's Assessment grid, select an assessment. Select Remove. Select Save & Close. Delete an Existing Assessment

CAUTION: Removing an item from the Today's Assessment grid does not resolve the diagnosis. The Add or Update Assessments template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates as the working assessment. If you remove one of multiple available assessments on the Add or Update Assessments template, you must select a new working assessment that carries over to the other templates that share the same working assessment.
