Open the Assessments Tab

There are several options for accessing the Add or Update Assessments template: On the SOAP template, in the Assessment / Plan panel, select Add/Update. Access the Add or Update Assessment Template

Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click Add/Update. The Add or Update Assessment template opens. Select an Assessment from Billing Diagnosis History

On the Add or Update Assessment template, Billing Diagnosis History section, select an existing diagnosis from the grid. To open the Common Assessment template, click Add common assessment. Add or Update Assessment The template opens to the page associated with the currently selected Specialty and gender and age of the patient. To view a different page, click the Common assessments field to open a picklist of assessment page options. Common Assessment Click Save & Close to close the Common assessment page. To open the diagnosis search window, click Diagnosis Code Lookup. Entering and Modifying Assessments

The Add or Update Assessment template is found on the Assessments tab, which addresses the needs of both ICD-10 coding and 5010 standards. The entered assessments display in a grid format to enable the submission of 12 diagnoses on a claim. The grid for Today’s Assessments on the Assessments tab contains the following elements: Today's Assessments First column (#) indicates the priority; important when more than 12 row items exist as only the first 12 can be submitted on the claim Second column holds the diagnosis description, diagnosis code, and if entered, the Status, Side, and Site Third column is a separated concatenation of the impression and differential diagnoses Note: If an HCC column appears in the grid, accessing the Problems module is required. Document an Assessment

To add or edit a diagnosis on the Add or Update Assessment template, use one of the following sections: Billing Diagnosis History Clinical Problems My Favorites The new or revised diagnosis appears in the list of diagnoses in the Today's Assessment grid. If a working assessment does not exist, the diagnosis information appears in the ICD description and ICD Code fields. If your practice required a status when you selected the diagnosis, it appears in the Status field. If a working assessment exists, it remains selected in the Today's Assessment grid, and the ICD description, ICD code, and status for the working assessment are retained. To change the working assessment, select the diagnosis being addressed for this encounter in the Today's Assessment grid. The selected diagnosis becomes the new working assessment that carries forward to the other templates that share the assessment. ICD Legacy Mapping

ICD Legacy Mapping enables you to view ICD-10 General Equivalency Mapping (GEM) codes for an ICD-9 code. These code mappings are drawn from the ICD-9-to-ICD-10 xref table supplied by Health Language, Inc. (HLI), a third party content provider. The diagnosis search window may be set up to launch in the ICD Legacy Mapping mode, or you can select an option for this mode on the window. Enter the ICD-9 code in the text box at the top of the diagnosis search window. Select ICD Legacy Mapping in the left pane. Select Search, or select the Enter key. The search results grid displays each unique ICD-10 code mapped to the ICD-9 code, which is shown in the Legacy Icd Code column. Note: The results show only the display term description of each unique ICD code, not every synonym for it. To view ICD code synonyms, see step 4. Right-click a row to display the following menu commands: Add to Favorite Group > Favorites group folder. Enables you to add the diagnosis to the selected Favorites group fold
