Convert ICD Codes in the Billing History

There may still be ICD-9 codes in the Billing Diagnosis History section for the open patient chart. The Assessments tab provides quick and easy conversion of historical data from ICD-9 to ICD-10. On the Add or Update Assessment template, Billing Diagnosis History section, select the ICD-9 code. The diagnosis search window opens. The selected code number appears in the search field, the ICD Legacy Mapping option is selected, and the ICD-10 codes mapped to the ICD-9 code appear in the search results grid. Do one of the following actions: Select any one of the mapped ICD-10 codes. Search for and select the desired code. Document an Assessment

To add or edit a diagnosis on the Add or Update Assessment template, use one of the following sections: Billing Diagnosis History Clinical Problems My Favorites The new or revised diagnosis appears in the list of diagnoses in the Today's Assessment grid. If a working assessment does not exist, the diagnosis information appears in the ICD description and ICD Code fields. If your practice required a status when you selected the diagnosis, it appears in the Status field. If a working assessment exists, it remains selected in the Today's Assessment grid, and the ICD description, ICD code, and status for the working assessment are retained. To change the working assessment, select the diagnosis being addressed for this encounter in the Today's Assessment grid. The selected diagnosis becomes the new working assessment that carries forward to the other templates that share the assessment. Entering and Modifying Assessments

The Add or Update Assessment template is found on the Assessments tab, which addresses the needs of both ICD-10 coding and 5010 standards. The entered assessments display in a grid format to enable the submission of 12 diagnoses on a claim. The grid for Today’s Assessments on the Assessments tab contains the following elements: Today's Assessments First column (#) indicates the priority; important when more than 12 row items exist as only the first 12 can be submitted on the claim Second column holds the diagnosis description, diagnosis code, and if entered, the Status, Side, and Site Third column is a separated concatenation of the impression and differential diagnoses Note: If an HCC column appears in the grid, accessing the Problems module is required. Add New Assessments From Common Assessments Page

If the assessments that you want to add are not in the Billing Diagnosis History grid, you can add them from the Add common assessment link below the grid. Select the Add common assessment link. The Common Assessment template opens. If the page is blank or shows a common assessment page other than the one you want to use, do the following: Click the Common assessments field. The Get Assessment Page picklist opens. Find the assessment page you need in the list. Select the page and click OK. The template shows the diagnoses and codes for the selected page. Select on all assessments that you want to add to today’s encounter. Assessments display in the grid. Select Save & Close . Assessments you select appear in the Billing Diagnosis History grid. For each assessment that you want to add to Today’s Assessment s, select it and then Click Add/Update. The assessment enters the Today’s Assessments grid on the Add or Update Assessment template. Create Assessment Pages

You can create assessment pages used to add diagnoses to a patient's history. On the Specialty Common Assessment Config template, expand the Manage Common Assessment Pages and Links panel. Note: By default, the active assessment pages are displayed in the grid. Enter the name of the assessment page to be created in the field for the Common Assessment Page column. From the field for the Category column, select the desired category. Note: The category determines whether the page is viewable on the Inf Common Assessment template when it is opened from medical record templates. From the field for the Age column, and select the appropriate age group. From the field for the Gender column, and select the appropriate gender. To set the assessment page as default to the current practice or a provider, select Practice Default or a provider's name from the field for the Rendering column. Add New Assessments Using the Diagnosis Code Lookup Link

If the assessments that you want to add are not in the Billing Diagnosis History grid, you can add them from the Diagnosis Code Lookup link below the grid. Select the Diagnosis Code Lookup link. The diagnosis search window opens. Do one or both of the following, as needed. Enter any string of characters that may be in the clinical description for the diagnosis, and click Search. Scroll to find the diagnosis in the right-hand list. Highlight the diagnosis. Select Select. The Diagnosis search window closes and the diagnosis you selected appears in the Billing Diagnosis History grid. Select Add/Update to add the diagnosis to Today’s Assessments. When all assessments are in the Today’s Assessments grid, click Save & Close. All assessments in the Today’s Assessments grid on the Add or Update Assessment template are entered into the Today’s Assessments grid on the MICS Med/Admin OPH template.
