Reviewing Patient History on SOAP

You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Review of Intake Details by Provider

You can start the visit by reviewing the patient's details entered by the intake staff on the Home Page template, to determine if any updates should be made. The Home Page template consists of the following panels. Panel Description Medical Chart Summary Displays the details documented by the intake staff and provider during an encounter. The details from the previous encounters also appear in this panel. The categories in the left pane in the grid allow you to select and view the documented data. Some of the categories include filters in the grid, which allow you to filter and review the required data. The data that appears in the grid is read-only. Problem List Displays the patient's chronic and secondary problems documented by the intake staff. History Summary Displays the patient history details previously documented by the intake staff and provider on the Intake, Histories, and SOAP templates. Reviewing Patient's General Information on SOAP

You can view the general details of a patient in the General panel on the SOAP template. In the General panel, you can do the following: Document the relationship of the person who provides the patient information, in the Historian field. Capture the electronic Summary of Care requests and receipts that are completed electronically, and provide the information as reportable fields on the Transitioning Into Care template. On the Addition Visit Information template, you can provide answers for the preconfigured questions in the Interpreter Details category. You must document interpreter details when someone else interprets the patient’s communication for you. Indicate if the patient for the current encounter is new or established. Patient Tracking in the Provider Workflow

You can update the patient tracking details using the Patient Tracking on the Patient Information Card (PIC). You can select the Patient Tracking button on the Patient Information Card (PIC) and update the tracking details on the Todays Patient Tracking template. Patient Tracking is available only when the patient has an active encounter dated Today. For all other encounters, the Patient Tracking button appears as New Encounter. You can hover over the Patient Tracking button to view a quick summary of the patient tracking records for the visit on the PIC. Before starting the patient assessment on the SOAP template, you can indicate that the patient is with the provider by changing the status to with provider in the Todays Patient Tracking template. When the patient assessment is completed, you can change the status to ready for check-out. View the Audit Log for Provider Communication

You can view edited or deleted provider communication records and the modifications made to the records on the Provider Communication Audit Log template. Open NextGen® Enterprise EHR. Open the SOAP template. Select Provider Comm .The Provider Communication template opens. To select a communication record, do one of the following: Select a record in the grid, and select the Audit Log link. The Provider Communication Audit Log template opens. The selected record appears in the Provider Communications grid. Select the Audit Log link to open the Provider Communication Audit Log template, and select a record in the Provider Communications grid. Any changes or updates to the selected communication appear in the Audit Log grid. In case of no changes, the Audit Log grid remains empty. Provider Communication Audit Log sections Note: On Provider Communication Audit Log, you can filter the records in the Provider Communications grid by using the options available in the Show filter. Select Close.

Provider Communications

Providers can document communications to the patient, family members, or providers on the Assessment/Plan - OPH template in the Provider Communication panel. A history of documented communications appears in the grid. The grid can be filtered to display communication records by status, active (includes edited records) or deleted, or all records. By default, the grid is filtered to show Active records. Fields on the Provider Communication panel are shared with the Provider Communication template.
