Patient Search Criteria Information

Field	Description
Last	Patient's last name or part of last name.
First / Preferred	Patient's first name or preferred name.
Middle	Patient's last name or part of middle name.
Suffix	Patient's last name or part of last name. Note: This field appears only if the Include suffix on lookup screens option is configured in Practice Preferences.
Previous Last	Patient's previous last name or part of previous last name.
City	Patient's city.
Address Line 1	Patient's street address.
Zip	Patient's zip code.
Mother's Maiden Name	Patient's mother's maiden name.
Social Security	Patient's social security number. Note: This field cannot be used with other search criteria.
Birth Date	Patient's birth date in MM/DD/YYYY format.
Sex	Patient's birth sex. The following options are available: Female Male Undifferentiated Unknown
Phone	Patient's 10-digit home phone number. Note: This field cannot be used with other search criteria.
Search By	Is based on Person Lookup Search By selected in Practice Preferences. The Search By list consists of the following search methods. The name of the field next to Search By changes to the name of the selected number type. Med Rec Nbr is the patient's medical record number. Person Nbris the patient's person number. Other ID Nbr is the patient's other ID number. Note: This field cannot be used with other search criteria. The field is not available on the People Lookup window in NextGen® Enterprise PM or NextGen® Enterprise EHR.
Policy Number	Patient's insurance policy number.
Enc Number	Patient's encounter number. Note: This field cannot be used with other search criteria. The field is not available on the People Lookup window in NextGen® Enterprise PM.
View By	You can view patients by a specified parameter. The following options are available: All Patients (default) Appointment is the appointment date and room Default Location is the default location Preferred Provider is the preferred physician Primary Care Provider is the primary care physician Provider is the rendering physician Provider Specialty is the rendering physician and specialty Provider Appointment is the rendering physician and room Referred By is the referring physician
External System	Used with the External ID field.
External ID	Patient's ID number in the selected external system. Note: This field is used with the External System field.
Birth Date (with padlock icon)	You can enter the patient's birth date in the MM/DD/YYYY format to find a patient in another association by performing a secure lookup. The Last name and First name fields must also have an entry. Note: This field appears only if the Enable Secure Lookup option is configured in System Administrator.
L4DSSN (with padlock icon)	You can enter the last four digits of the patient's social security number to find a patient in another association by performing a secure lookup. The Last name and First name fields must also have an entry. Note: This field appears only if the Enable Secure Lookup option is configured in System Administrator.
Client Defined	An enterprise-level client-defined field can be displayed and used as a patient lookup parameter if it is set up in the Enterprise Preferences Client Defined tab. For more information on setting up enterprise-level client defined fields, go to NextGen Healthcare Success Community, and download the latest File Maintenance Preferences Guide for NextGen Enterprise.
Exclude Expired Patients	You can specify if you want to exclude or include expired patients in your search results. You can select the check box to exclude expired patients in your search. You can clear the check box to search for expired patients as well as living and active patients. Note: The default setting for this check box is controlled by the Exclude expired patients default option on the General tab of Practice Preferences.
Phonetic Search	You can specify if you want to search for patients with names sounding phonetically similar in the search results. The results are weighted so that the closest match appears at the top of the search results list. Note: This check box appears only if the Display phonetic search option is configured on the General tab of Practice Preferences . The default setting for this check box is controlled by the Use phonetic search option in your General User Preferences.
Orders	Orders search parameters appear only on the Patient Lookup window in NextGen® Optical Management. The following options are available: None (default) EyeGlass : Enter the Order Number, Job Type, or Start/End Dates Contacts : Enter the Order Number or Start/End Dates Accessories : Enter the Order Number or Start/End Dates
