The following table describe the person or patient search criteria fields on the People Lookup window.
| Search Criteria | Description |

| Last, First / Preferred, Middle, Suffix | Searches by the patient's name. You can enter the last name, first name (or preferred name), middle name, suffix, or any combination of a patient's name.

| Previous Last | Searches by the previous last name if the patient's last name has changed. |
| City | Searches by the city of residence. |
| Address Line 1 | Searches by the patient's street address. |
| Zip | Searches by the patient's zip code. |
| Mother's Maiden Name | Searches by the maiden name of the patient's mother. |
| Social Security | Searches by the patient's Social Security Number. |
| Birth Date | Searches by the patient's date of birth. |
| Sex | Searches by the patient's gender. |
| Home Phone | Searches by the patient's home phone numbers |

| Policy Nbr | Searches by patient's insurance policy number. |
| Enc Nbr | Searches by an encounter number associated with a patient. |
| View By | You can specify how you want the results of your search to appear. You can sort the results in any of the following orders:

| External System | Searches using a third-party application that interfaces with NextGen® Enterprise PM and stores patient chart records. |
| External ID | Searches using the ID used by the selected External System to identify a patient. External ID number may differ from the ID number provided by NextGen® Enterprise PM. |
| Exclude Expired Patients | Excludes patients who are no longer living. |
| Birth Date | Search by the patient's date of birth. You can enter the date manually or select the date from the pop up calendar. Birth Date is a secure lookup field. |
| L4DSSN | Search for a patient using the last 4 digits of his or her Social Security Number. L4DSSN is a secure lookup field. |
| Chart Status - Home Lock | Search by selecting either Active or Inactive for the chart status. Chart Status- Home Lock narrows down the search. |
| Phonetic Search | Select this option to perform a phonetic search for a patient lookup. A phonetic search returns results which include names that sound similar to the name you entered as a search criteria.
