PT/OT Modalities Skin Check Completed Checkbox
The Modalities template has a Skin Check Completed checkbox in the lower right. Skin Check Completed checkbox highlighted on Modalities template If you mark Required here, if therapist has documented that a modality has been completed (for example, Electrical Stimulation or Hot/Cold Therapy), the therapist cannot submit charges on that encounter until they mark the Skin Check Completed checkbox. If you select Not Required, marking the checkbox is optional. If you select Hidden, the Skin Check Completed checkbox will not appear on the Modalities template. Modalities
You can document all the modalities completed with the patient on the Modalities or Additional Modalities template. You can document following modalities on the Modalities template. Hot/Cold Therapy (97010) Ultrasound (97035) Iontophoresis (97033) Fluidotherapy (97022) Vasopneumatic Device (97016) Electrical Stimulation (97014) Modalities template If you perform a modality at the same time as another treatment that you are billing for, select the corresponding Concurrent with exercise checkbox to suppress the billing. The selected modality appears on the Fee Sheet as Concurrent and Not Billed. You must enter the time for all the documented modalities in the corresponding Total Min or Min fields. If an assistant performed the therapy, then you can enter their time in the Asst Time fields. The time entered for all the modalities is combined and displayed in the Total time field. You can select the Attended checkbox in the Electrical Stimulation section to indicate that the therapist was

Electronic Clinical Summary
You can use the Electronic Clinical Summary section to document if the patient Accepted, or Declined an electronic clinical summary or requested a paper copy (Requested Paper). Select Visit Type Complexity
For visits that require a complexity code (PT/OT) or an E&M code (acupuncture), options appear below the Visit Type. Select the appropriate option.
