The Pharmacy field on the Intake template displays the patient’s preferred pharmacy. You can add or update a pharmacy from the Intake template or the Modify Patient Information template. Patient is Accompanied By

Use the Patient is accompanied by field to note who accompanied the patient during the visit. This is a multi-select picklist. The picklist remains open until you close it, allowing you to add multiple people. Patient is accompanied by field on the Intake template Click the Patient is accompanied by field to select who is accompanying the patient. The Accompanied By picklist opens. Record NextGen Portal Registration

The NextGen Portal Registration section of the Intake template enables you to see if the patient is activated in the portal, and the email address associated with the portal. NextGen Portal Registration section of the Intake template Select one of the following options for portal registration: Enrolled Declined Note: The Activated option is automatically selected if the patient is active in the portal. The email address automatically pulls from the NextGen® Enterprise PM application. If you are enrolling the patient while they are present in the room, select the In Room check box. Summary of Care Requests and Receipts

You can open the Transitioning Into Care template which records electronic Summary of Care requests and receipts as reportable fields on the template. The Transitioning Into Care template indicates whether a summary of care records for a specific patient are requested electronically and if any summary of care records are received. You can also document the completion of a Summary of Care request without a document returned. The Transitioning into Care link is available in the General panel of the templates. The information icon next to the Transitioning Into Care template provides basic template instructions. You can use the Clinical Reconciliation link to open the Clinical Reconciliation module. You can access the actual Summary of Care documents through a patient’s chart. These documents are available on the Categories tab. Enter the Patient's Disposition

You can document the disposition of a patient in the disposition section of the My Custom Plan template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the specialty as Family Practice and the visit type as Office Visit, and then select My Plan.The My Custom Plan window opens.
