View and Add Summary of Care Documents Received

You can view and add details of summary of care documents received. The Received/Requested Summary of Care section includes the following two options, and an [*] asterisk indicates the current option being reviewed in the grid:: Summary of Care Received Summary of Care Requests The Summary of Care Received grid contains the last 18 months of Summary of Care documents details imported from a third-party Health Information Exchange (HIE) and are a part of the patient’s chart. The documents may include an original request from the provider or clinical staff. The documents may also imported through HIE without a request. Open NextGen® Enterprise EHR. On the Intake or SOAP template, expand General. Select the Transitioning Into Care link. The Transitioning Into Care template opens. Select Summary of Care Received. Add the Summary of Care Details in the Associated Requests and /or Summary of Care Documents Received Grid

Using the Summary of Care Received option, you can add the Summary of Care document details automatically and manually in the Associated Requests and/or Summary of Care Documents Received grid. NextGen® Enterprise EHR automatically fills the Associated Requests and/or Summary of Care Documents Received grid with data from the Summary of Care document when the document is associated with the current encounter. This current encounter details are stored during the request functionality and/or the clinical reconciliation process. Each Summary of Care document details display as a separate row in the grid. A single Summary of Care document can satisfy multiple transitioning into care encounters. If the patient is enrolled in Enterprise Chart, you can see Summary of Care documents from different practices. Open NextGen® Enterprise EHR. On the Intake or SOAP template, expand General. Transitioning into Care

You can select the Transitioning into care checkbox to indicate that a transitioning into care encounter is established. The Transitioning into care checkbox on the Transitioning Into Care template is automatically selected when one of the following criteria is met: A Summary of Care document that contains today’s encounter details is automatically added to the Associated Requests and/or Summary of Care Documents Received grid. A Summary of Care document is manually added to the Associated Requests and/or Summary of Care Documents Received grid. The Received/Requested Summary of Care section includes the following two options: Summary of Care Received Summary of Care Requests An [*] asterisk next to either the Summary of Care Received or Summary of Care Requests fields indicates the current option being reviewed in the grid. The Summary of Care requests grid contains the last 18 months of Summary of Care requests associated with the patient's Summary of Care documents. The Summary of Care Requests section provides you the flexibility to select a single request and associate a single, multiple, or all documents with the Associated Requests and/or Summary of Care Documents Received grid. The Summary of Care documents grid contains the last 18 months of Summary of Care documents imported from a third-party, Health Information Exchange (HIE) and are a part of the patient’s chart. The documents may include an original request from the provider or clinical staff. The documents may also be imported through HIE without a request. Open

Clear the Transitioning Into Care Check Box

You can manually clear the Transitioning into care check box. To clear the Transitioning into care check box on the Transitioning Into Care template for an encounter, the following conditions must be met: The Associated Requests and/or Summary of Care Documents Received grid on the Transitioning Into Care template does not contain any Summary of Care document details. The Summary of Care documents received or queried check box is not selected. To manually clear the Transitioning into care check box, remove all the entries from the Associated Requests and/or Summary of Care Documents Received grid. Open NextGen® Enterprise EHR. On the Intake or SOAP template, select General . Select the Transitioning Into Care link. The Transitioning Into Care template opens. In the Associated Requests and/or Summary of Care Documents Received grid, select all the rows, and then select Remove. Clear the Transitioning into care check box. Viewing the Associated Requests and Summary of Care Documents Received Details

You can view the Summary of Care document details in the Associated Requests and/or Summary of Care Documents Received grid. The Associated Requests and/or Summary of Care Documents Received grid determines which of the following two checkboxes get automatically selected based on the details available in the grid. The Summary of Care document requested or queried checkbox is selected automatically when: The patient has at least one Summary of Care request or query within the last 18 months of the encounter. You add a Summary of Care received details and its associated document with a Summary of Care request. You can also manually select or clear the Summary of Care document requested or queried checkbox when: A request for Summary of Care document is made outside the NextGen Healthcare system.
