Patient History for Post-Operative Visits

You can associate surgical procedures with post-operative visits, and document a patient's history for post-operative visits. On the Non Billable Visit template, in the History panel, you can associate surgical procedures to the post-operative visit. The Past Orthopedic Surgeries grid in this panel displays the details of the patient's previous surgeries. You can document the following details of the patient for the post-operative visit. Last injection Pain Associated symptoms Ambulatory aids and braces Function and activity levels Sports and hobbies Employer details Medications and allergies History panel on the Non Billable Visit template You can use the Overflow Type section to add additional symptoms, ambulatory aids and braces, and function and activity levels if the fields in their respective sections are insufficient. Assessment and Plan for Non-Billable Post Op Visits

The Assessment and Plan panel allows the provider to document a post-op visit for an initial fracture care procedure, the plan, verbal counseling, and next visit information. The Diagnosis grid displays the patient’s diagnoses. The system defaults to the Orthopedic option to view Orthopedic-specific diagnoses. The Medical option displays non-orthopedic diagnoses. Non-Billable Post-Op Visit Workflow

The Non-Billable Visit template is for post-op visit documentation, as well as, any non-billable visits that require HPI, Exam, and Plan information. The Non-Billable Visit template has four panels; History, Infection, Physical Exam, and Assessment and Plan (MDM). Access the Non-Billable Visit Template You can access the Non Billable Visit template from the Home template. Non-Billable Fracture Care Post-Op Workflow

Each fracture care template has a corresponding non-billable fracture care template such as NBV Single Fx, NBV Multiple FX, and NBV Poly-Trauma FX follow-up. The non-billable fracture care templates are accessed by clicking the Non-Billable tab and selecting the desired template. These templates enable you to document a follow-up visit for the initial fracture care visit. Each of these templates has four panels; Surgical Information, History, Physical Exam, and Assessment and Plan. Copy Previous Surgery Details You can copy details of previous surgeries for assessment and planning of the polytrauma fracture care on the NBV PolyTrauma template. Billable Visits

The Billable Visit workflow starts on the Billable Visit template. This template is used to document the History of Present Illness (HPI), Physical Exams, and Assessment and Plan portions of an E&M coded visit. It pulls in information from the Intake template. It has three panels: History, Physical Exam, and Assessment and Plan. Documenting Visit Time You can enter the visit time for a patient's visit on the Billable Visit template. Patient History You can review, add, and update the patient histories. Physical Exam Panel Assessment and Plan Panel (Diagnosis/MDM) Billable Visit Sign-off Documenting Billable Visit Counseling You can document the additional counseling time and its associated details for a patient. Finalizing a Patient Visit You can enter a patient's visit details and today's diagnoses, and apply diagnosis codes to procedures to submit charges and finalize the visit. Exams

To open an exam, click on its name in the left side navigation pane. Each exam (body area) has its own template. Exams that are most pertinent to the current workflow are shown at the top of the list. You can scroll down to see additional links. Data entered on exam templates is shown in the large grid below the Vitals information on the Physical Exam panel. Each exam template has side navigation to open related templates. For example, shown directly below is the template page that opens when you click the Fracture link. Note that links to other fracture templates are at the top of the left navigation bar. Exam template All exam templates have the QuickPick option to fill in information, as well as the Get Data functionality to pull data forward from previous encounters with the patient. Each physical exam template has an eraser icon for the whole template, and for the specific sections. The eraser icons enable you to erase a portion of the exam or the entire exam.
