Add an Assessment (Diagnosis)

The Today’s Diagnosis grid pulls in diagnoses that were entered on the Billable Visit template under the Diagnosis Management section. Diagnoses with a check mark are selected to be applied to Procedure codes in the coding grid. All diagnoses are selected by default when you open the Finalize Visit template. Click the Add - Remove button below the Today’s Diagnosis grid. The IORTS FV Add Assess template opens. IORTS FV Add Assess template Double-click the diagnosis in the patient’s Diagnosis grid to add it to the Today’s Diagnosis grid. Note: If a diagnosis is not in the grid, you can find and add it by using the Add button (or the Problem IT button if your practice is configured to use IMO). Letters and Reports on the Finalize Visit Template

The Finalize Visit template has links to letters, office notes, and reports. The Radiology Report displays all non-X-ray orders that have been performed and interpreted on today's encounter. The X-ray Report displays all x-ray tests that have been performed and interpreted on today’s encounter on the Radiology Order Management template. There is also a Document Generation Log, carbon copy provider fields, Electronic Clinical Summary, and NextGen Portal Registration sections. You can click the report and letter icons to generate documents. The Electronic Clinical Summary and NextGen Portal Registration selections carry forward if entered on the Intake template. Document That the Encounter is Incident to a Previous Encounter

On the Finalize Visit template, select the Incident To check box. The IORTS Incident to Plan window opens. You can filter the grid by selection the following option: Time Period Current Provider All Providers All Encounters Plans Only-ones that had plans Add additional diagnoses in the Today's Diagnosis grid if your practice is configured to allow it. Note: If new diagnoses are added in this manner there must be documentation on the Billable Visit template per CMS guidelines. If the Add New Task window opens, do one of the following: To close the window without sending a task, click Cancel. To send a task, select any desired recipients (for example, billing department), move them to the Task Recipients grid, and then click Task. Incident To Settings

The Incident To settings section enables you to set defaults to filter the options for Incident To billing. These settings affect what is displayed in the IORTSincident To Plan window that opens on selecting the Incident To check box on the Finalize Visit template. Document General Information of Patients

You can document a patient's general information on the Intake template. When someone provides the patient details, you can document the historian details in the Historian field. When someone interprets the patient’s communication, you can select the Interpreter Details link to document the details of the interpreter. You can select Configurable Questionnaires link to document the screening details of the patient. You can select the Transitioning into Care link to document the summary of care record information for a patient.
