Access the Patient Demographics Template

You can also access the Patient Demographics template from Demographics links on various NextGen® Adaptive Content Engine. For more information, go to the NextGen Healthcare Success Community, and download the NextGen® Adaptive Content Engine documentation for the specific template. Open an existing patient in NextGen® Enterprise EHR. For more information, refer to the Person and Patient Lookup in EHR section. Open the Select Template window and select the Demographics tab. Select the Patient Demographics template. For more information on selecting a template, go to the NextGen Healthcare Success Community and download the Templates User Guide for NextGen Enterprise EHR. The Patient Demographics template appears, displaying the selected patient's information. Expand the panels to view the information in them. You can add or edit information in the enabled fields. To save your updates without closing the template, select Save from the File menu. To save the updates and close the templat

Demographic Information Panel

This panel displays patient demographic information pulled from NextGen® Enterprise PM. Demographic Information panel on the Pre Reg template You can update the patient SSN, and Address by clicking in the appropriate fields. Any changes made in this panel will be updated in NextGen® Enterprise PM. If the patient is a minor, enter the guardian’s name and relationship. To launch the Authorization to Treat template, right click the grid and select Add New. Employer information can be added by clicking the Add button below the employer grid and filling out the patient information. Phone numbers and best contact time can be entered as well as ethnicity, race and language. Double-click the appropriate grid to launch the templates or click the ELR button to launch the Patient Information template. Any changes made in these panels are sent to NextGen® Enterprise PM. Open the Patient Demographics Template

You can document patient details such as contact number, address, Primary Care Physician (PCP), primary practice provider, insurance, pharmacy, provider, emergency contact, employment, and reminder details, on the Patient Demographics template. Open NextGen® Enterprise EHR. Select a patient record. To open the Patient Demographics template, do one of the following: Open the Templates module, and double-click Patient Demographics in the Demographics tab. On the Home Page template, select the Demographics link. The Patient Demographics template opens. Patient Demographics Add Patient Demographic Information You can add demographic details of a patient on the Patient Demographics template. Editing the Patient's Address History You can view and edit the patient's address history details in the Address panel of the Patient Demographics template. Patient Demographics

You can review a selected patient's demographic information in the NextGen® Adaptive Content Engine Patient Demographics template. You can also add or edit the default pharmacy, historical addresses, reminders, emergency contacts, and employment. To update other patient information, use the Modify Patient Information window. To add a patient, use the Add Patient Information window. For more information on person and patient search and to view patient information, refer to the Person and Patient Lookup in EHR and the Person and Patient Information sections. Add Patient Demographic Information

You can add demographic details of a patient on the Patient Demographics template. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Patient Information panel. Patient Information Enter values for the following fields: State of birth Country of birth Blood type Enter information in the Race and Ethnicity fields. Note: You can add multiple values for race and ethnicity. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields. The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template If the patient is married, enter the spouse name. Add Reminders through the Patient Demographics Template

You can add a reminder. You can add and view reminders associated with patients in the Reminder panel of the Patient Demographics template. Patient Demographics Open NextGen® Enterprise EHR. Open the Patient Demographics template, expand the Reminder panel. Select the reminder type, method of contact, and send date, and enter your comments for the reminder. Select Add. The reminder is added to the Reminder panel grid, where you can view the reminder type, method of contact, type of sender, language of contact, and outreach campaign name associated with the patient.
