Add Patient Demographic Information

You can add demographic details of a patient on the Patient Demographics template.

1. Open NextGen® Enterprise EHR.
2. Open the Patient Demographics template, and then expand the Patient Information panel.
   

3. Enter values for the following fields:
   • State of birth
   • Country of birth
   • Blood type
4. Enter information in the Race and Ethnicity fields.
   Note: You can add multiple values for race and ethnicity.
5. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields.
   The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template.

   

6. If the patient is married, enter the spouse name.
7. Select Update, and then select Save & Close.
