Add Patient Demographic Information

You can add demographic details of a patient on the Patient Demographics template. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Patient Information panel. Patient Information Enter values for the following fields: State of birth Country of birth Blood type Enter information in the Race and Ethnicity fields. Note: You can add multiple values for race and ethnicity. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields.The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template. Open the Patient Demographics Template

You can document patient details such as contact number, address, Primary Care Physician (PCP), primary practice provider, insurance, pharmacy, provider, emergency contact, employment, and reminder details, on the Patient Demographics template. Open NextGen® Enterprise EHR. Select a patient record. To open the Patient Demographics template, do one of the following: Open the Templates module, and double-click Patient Demographics in the Demographics tab. On the Home Page template, select the Demographics link. The Patient Demographics template opens. Add a Patient's Previous Address

You can add a previous address for a patient. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Address panel. Address In the Address History section, enter the start and end dates to indicate the duration of residence at this address. Note: By default, the Type field is historical. Enter the patient's previous address and phone number. Enter the ZIP code. The city, state, county, and country information fields are automatically filled. Managing the Telephone Call Template

You can record information gathered during a telephone conversation with the patient. The Telephone Call template and the Telephone Communication template enable you to record information gathered during a telephone conversation. You can use the Call/Communication sub-navigational link to open the Telephone Call template. Update or Delete a Reminder

You can modify reminder details for a patient on the Telephone Call template. Open NextGen® Enterprise EHR. On the Telephone Call template, expand the Reminder panel. Select a reminder from the grid. To update the reminder details, modify the details, and then select Update. The updated reminder appears in the grid. To delete the reminder details, select Remove. Add Reminders through the Telephone Call Template

You must use clinical relevant information to identify patients who must receive reminders for preventive or follow-up care. To meet the Quality Reporting requirement and receive a Quality Reporting credit on the Telephone Call template, you must only send reminders based on the patient's preferred time and method of contact. A patient's decision to opt out of reminders can be recorded, and that patient can still be counted toward the Quality Reporting requirement. Open NextGen® Enterprise EHR. In the Telephone Call template, expand the Reminder panel. To add the reminders, do the following: Note: If the patient does not want reminders, you can select the Patient declines reminders checkbox, and then save and close the template. Select a reminder type. In Contact method, select Preferred or Other, and then select a method. In the Sent field, select a date. Enter additional comments related to the patient reminder.
