Remove an Assessment from Today's Assessments

Add a New Assessment Using the Diagnosis Search

You can search for an assessment using the diagnosis search window. Click the Diagnosis Lookup link. The diagnosis search window opens. diagnosis lookup link Enter the first few letters of the desired assessment in the Search field. Click Search. The results grid refreshes to display a list of assessments that include the searched for item. Highlight a row in the results grid. Click Select. The selected assessment appears in the Today's Assessments grid. Update the Assessment Details in Today's Assessments grid

If you want to update the assessment details in the Today's Assessments grid, Highlight an assessment in the Today's Assessments grid. Select or enter appropriate value in the Status, Side, Site, Impression, Differential Dx fields. Select Update. If the diagnosis is chronic, select the Mark diagnosis as chronic checkbox. If you want to add the selected assessment to the problem list, tracked problems, and/or favorite list, select the Clinical problems, My tracked problems, and/or My favorites checkbox respectively. Note: You can select the other tabs such as My Plan, A/P Details, and so on to document treatment plan, prescribe medications, order labs or X- rays, and request referrals. Add New Assessments Using the Diagnosis Code Lookup Link

If the assessments that you want to add are not in the Billing Diagnosis History grid, you can add them from the Diagnosis Code Lookup link below the grid. Select the Diagnosis Code Lookup link. The diagnosis search window opens. Do one or both of the following, as needed. Enter any string of characters that may be in the clinical description for the diagnosis, and click Search. Scroll to find the diagnosis in the right-hand list. Highlight the diagnosis. Select Select. The Diagnosis search window closes and the diagnosis you selected appears in the Billing Diagnosis History grid. Select Add/Update to add the diagnosis to Today’s Assessments. When all assessments are in the Today’s Assessments grid, click Save & Close. All assessments in the Today’s Assessments grid on the Add or Update Assessment template are entered into the Today’s Assessments grid on the MICS Med/Admin OPH template. Add Diagnostic Details

In the Assessment/Diagnosis panel, confirm the assessment selected displays in the Description details and ICD Code field. If not, select the assessment in the grid to fill in the details. Add Status, Severity, Differential Dx, and Impression. To enter specifiers in the Specifiers field, do one of the following: Select in the field and enter free text. Select the My Phrases link and select a phrase from the list. If applicable, select Present on Admission and then select a description from the list. To indicate the diagnosis is chronic, add the diagnosis to the client’s clinical problems list, and/or add the assessment to my tracked problems, select the respective checkbox. Select Add To Grid. The information displays in the grid. To add psychosocial stressors, select the Psychosocial Stressors link. To add referrals, select the Referrals link. To reorder the diagnoses, select Sort. Grids in Add or Update Assessment Template

You can add or update the assessment template. The Add or Update Assessment template includes the following grids. Grid Description Today’s Concerns/Reasons for Visit Displays the concern or reason of visit. Billing Diagnosis History Displays a list of active diagnoses for this particular patient. Clinical Problems Displays a list of all active problems that have been entered into the Problems module for this patient.
