Upload the Procedure Diagnosis in Today's Assessment Grid

In the Documentation and/or Findings panel, select the values that are associated with the procedure diagnosis. Expand the Assessment/Plan panel and select the Auto Dx button.The Add or Update Assessment template open. The system uploads the procedure diagnosis in the Today Assessment grid. Add or Update Assessment Update the assessment details in the Today's Assessments grid. Select Save & Close.The diagnosis details appear in the grid. Update an Assessment in Today's Assessment Update an Assessment in Today's Assessment You can update the patient's assessments in the Today's Assessments grid. You can also add an assessment to the patient's clinical problems list as a chronic condition or add it to the provider's list of tracked problems and My Favorites so that it can easily be found and selected for other patients. Highlight the row with the assessment in the Today's Assessments grid. The fields in the Impression section fill with the selected assessment details. Click the Side field and select the affected side from the pick list. The selected value appears in the Today's Assessments grid and the Impression section. Enter a Status. To add the assessment to another grid, select one of the following for Add to: Clinical Problems includes both chronic conditions and tracked problems. Add Diagnostic Details

In the Assessment/Diagnosis panel, confirm the assessment selected displays in the Description details and ICD Code field. If not, select the assessment in the grid to fill in the details. Add Status, Severity, Differential Dx, and Impression. To enter specifiers in the Specifiers field, do one of the following: Select in the field and enter free text. Select the My Phrases link and select a phrase from the list. If applicable, select Present on Admission and then select a description from the list. To indicate the diagnosis is chronic, add the diagnosis to the client’s clinical problems list, and/or add the assessment to my tracked problems, select the respective checkbox. Select Add To Grid. The information displays in the grid. To add psychosocial stressors, select the Psychosocial Stressors link. To add referrals, select the Referrals link. To reorder the diagnoses, select Sort. Add New Assessments Using the Diagnosis Code Lookup Link

If the assessments that you want to add are not in the Billing Diagnosis History grid, you can add them from the Diagnosis Code Lookup link below the grid. Select the Diagnosis Code Lookup link. The diagnosis search window opens. Do one or both of the following, as needed. Enter any string of characters that may be in the clinical description for the diagnosis, and click Search. Scroll to find the diagnosis in the right-hand list. Highlight the diagnosis. Select Select. The Diagnosis search window closes and the diagnosis you selected appears in the Billing Diagnosis History grid. Select Add/Update to add the diagnosis to Today’s Assessments. When all assessments are in the Today’s Assessments grid, click Save & Close. All assessments in the Today’s Assessments grid on the Add or Update Assessment template are entered into the Today’s Assessments grid on the MICS Med/Admin OPH template. If the assessments that you want to add are not in the Billing Diagnosis History grid, you can add them from the Add common assessment link below the grid. Select the Add common assessment link. The Common Assessment template opens. If the page is blank or shows a common assessment page other than the one you want to use, do the following: Click the Common assessments field. The Get Assessment Page picklist opens. Find the assessment page you need in the list. Select the page and click OK. The template shows the diagnoses and codes for the selected page. Select on all assessments that you want to add to today’s encounter. Assessments display in the grid. Select Save & Close . Assessments you select appear in the Billing Diagnosis History grid. For each assessment that you want to add to Today’s Assessment s, select it and then Click Add/Update. The assessment enters the Today’s Assessments grid on the Add or Update Assessment template. Add a New Assessment Using the Diagnosis Search

You can search for an assessment using the diagnosis search window. Click the Diagnosis Lookup link. The diagnosis search window opens. diagnosis lookup link Enter the first few letters of the desired assessment in the Search field. Click Search. The results grid refreshes to display a list of assessments that include the searched for item. Highlight a row in the results grid. Click Select. The selected assessment appears in the Today's Assessments grid.
