Update Ophthalmology Patient Providers

To access the Patient Providers – OPH template, do one of the following: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. To update the P.C.P. field, do the following: Click the field associated with the P.C.P. to open the Modify Patient Information window. Click the arrow by the Primary Care Provider field and select a value from the list. Click OK. For the following provider fields, click the field and select a name from the list: Referring Provider Specialist CC Provider 1 CC Provider 2 CC Provider 3 Click Save & Close. Generate Standard Ophthalmic Letter

You can generate two different standard letters on the Document Library - OPH template. You can open the Document Library - OPH template in the following areas: Select the Documents link in the left side navigation. Click the Document Library sub-navigational link. Select one of the following: Letter 1 Letter 2 Click Letter recipients and CC to select the recipients for the letter. Select one of the following letters: Standard initial visit letter Standard follow up letter Standard diabetic exam letter Generic letter to patient The system generates the selected letter. Generate Ophthalmic Referral Letters

From the Referral Letter Generator – OPH template, you can generate a referral letter to send to the patient’s primary care provider, referring provider, or other specialists. You can generate three different referral letters as needed. Click the Document Library link and then click the Referral letter generator link.The Referral Letter Generator – OPH template opens. referral letter generator - oph The read-only fields display the patient’s providers documented in the NextGen® Enterprise PM. In the Introduction section, select one of the following options to generate introductory text. Ophthalmology Document Library

You can generate formatted letters and documents such as letters to referring providers, consent forms, prescriptions, and discharge instructions on the Document Library - OPH template. The Document Library link opens the Document Library - OPH template. You can use the links in the Clinical Narratives (CCDA) section to create plain text notes for USCDI certification for the eight clinical note types. Add a Provider to the CC list

You can add or update the PCP, Referring or shared provider in the CC list. Click the field for PCP, Referring or CC Provider #. Providers this Encounter- to include the PCP and/or Referring provider in the CC list or update their details. Providers to Copy on Today’s Chart Note- to include shared providers in the CC list and add or update their details. Patient Providers template The Patient Providers template opens. Select the check box for a provider who needs to receive copies to select provider names. Enter the details for the provider in the fields. Submit a Request for Referring Physician Sign Off

On the Objective tab, navigate to the Documents and Printing panel. Select the Submit to referring physician for review check box. Note: Referring physician sign-off is only requested on evaluation and re-evaluation encounters and is used only by PT/OT providers who are in the same practice as a medical provider who referred the patient. The Select Task Recipients window opens. Select Task Recipients From the left pane of the Select Task Recipients window, select one or more users listed under the appropriate practice(s).
