Select Letter Recipients

You can select one or more providers to receive the referral letter on the Patient Providers - OPH template. You can access the Patient Providers – OPH template in the following locations: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. Select one of the Address letter to options to indicate the primary recipient of the letter. For example, select Address letter to P.C.P. to send the letter to the primary care provider. Note: The options are used to indicate the primary recipient of the letter. Select one or more check boxes to indicate the carbon copy recipients of the letter. Update Ophthalmology Patient Providers

To access the Patient Providers – OPH template, do one of the following: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. To update the P.C.P. field, do the following: Click the field associated with the P.C.P. to open the Modify Patient Information window. Click the arrow by the Primary Care Provider field and select a value from the list. Click OK. For the following provider fields, click the field and select a name from the list: Referring Provider Specialist CC Provider 1 CC Provider 2 CC Provider 3 Click Save & Close. Generate Ophthalmic Referral Letters

From the Referral Letter Generator – OPH template, you can generate a referral letter to send to the patient’s primary care provider, referring provider, or other specialists. You can generate three different referral letters as needed. Click the Document Library link and then click the Referral letter generator link.The Referral Letter Generator – OPH template opens. referral letter generator - oph The read-only fields display the patient’s providers documented in the NextGen® Enterprise PM. In the Introduction section, select one of the following options to generate introductory text. Ophthalmology Document Library

You can generate formatted letters and documents such as letters to referring providers, consent forms, prescriptions, and discharge instructions on the Document Library - OPH template. The Document Library link opens the Document Library - OPH template. You can use the links in the Clinical Narratives (CCDA) section to create plain text notes for USCDI certification for the eight clinical note types. Ophthalmology Drawings

Ophthalmic providers can create drawings to document exam findings. Links to the ophthalmic images appear at the bottom of the Provider - OPH template or can be accessed by clicking the Draw link. After the provider draws the exam findings, they are saved into the patient's chart and appear in the History Toolbar. Document General Patient Information

In the General panel, select New patient or Established patient. The patient type is required for billing purposes and determines the type of visit code calculated on the Finalize tab. If the patient has been seen previously, the system automatically selects Established patient. Click the Historian field and select a value from the list or select the top blank line to type a different value. To enter a different date for First seen, click the field and select a date using the Calendar. For Vision Correction, select Yes or No. If yes, enter the Type and Years for the following: Glasses Contact Lenses Select OD or OS for Dominant Eye.
