Generate Ophthalmic Referral Letters

From the Referral Letter Generator – OPH template, you can generate a referral letter to send to the patient’s primary care provider, referring provider, or other specialists. You can generate three different referral letters as needed.

Click the Document Library link and then click the Referral letter generator link.

The read-only fields display the patient’s providers documented in the NextGen® Enterprise PM.
In the Introduction section, select one of the following options to generate introductory text.
- Initial visit
- Follow up visit
- Opening text 1
- Free text
To include History of Present Illness (HPI) information in the letter, select the desired rows in the Include HPIs grid.
For the following sections, select from the check boxes above the grid to indicate the information to include in the referral letter.
- Visual Acuity
- Intraocular Pressure
- Past ocular history - disease
- Medical/Surgical History
To include diagnostic test interpretations, select the from the following:
- External exam
- Slit lamp exam
- Posterior exam
- Test interpretations- adds all diagnostic test interpretations from today's visit with a completed status.
Select a row from the Impression and Plan grid.
Select the Additional discussion check box and type your comments in the corresponding field.
In the Closing Text section, select one of the following options to generate closing text.
- Initial letter closing
- Follow up closing
- Closing text 1
- Free test - Enables you to type closing sentences into the Closing text field.
To select the referral letter recipients, click the Referring Providers link and select a provider on the Patient Providers – OPH template.
From the Letters section, select First letter and enter the following:
- Enclosed tests
- Sending instructions
- Letter sent by and Date
Click Generate Note.
To generate another letter, return to the Referral Letter Generator - Oph and select Second letter or Third letter and then click Generate Note.

Click the Document Library link and then click the Referral letter generator link.
