Information Documented for a Contact Lens Evaluation

The Contact Lens Evaluation template includes the following panels and sections where information can be documented for the contact lens fitting and evaluation visit:

Reason for VisitDisplays previously documented reason for visit and HPI information for the encounter; this information can be reviewed and updated; this is the same panel as on other templates.
Visual AcuityDisplays previously documented exam findings; a new exam can be documented; this is the same panel as on other templates.
General InformationUsed to document specific patient information related to wearing contact lenses.
Contact Lens History sectionDisplays historical information for up to two prescriptions; each prescription is displayed in separate grids for each eye.
Current Contact Lens Prescription sectionUsed for the prescription of the contact lenses the patient is currently wearing and to evaluate how the patient is doing with the current prescription.
Slit Lamp Evaluation sectionEnables you to document a brief exam or view findings from an exam performed previously during the encounter, such as on the Oph EyeTech template.
Contact Lens Fit AnalysisEnables providers to document how a contact lens fits a patient for multiple trial lenses. Contact lens prescriptions and findings are saved to separate grids for OD and OS. Contact lens prescriptions can be selected from the grids and copied to the prescription fields in the Contact Lens Prescription section.
Contact Lens Prescription sectionEnables providers to review and update up to two prescriptions for contact lenses and select details for the contact lenses to be dispensed or ordered for the patient. The prescription can be for trial lenses or the final contact lens selection. Note that trial lenses do not appear in the printed prescription since they are dispensed but not yet prescribed.
Lens(es) Dispensed/To Be Ordered sectionUsed to document details for lenses being ordered or dispensed, along with instructions to the patient for wearing and care, and instructions for the order. Billing codes are also selected here, which are submitted on the Finalize – OPH template.
