Information Documented for a Contact Lens Evaluation

The Contact Lens Evaluation template includes the following panels and sections where information can be documented for the contact lens fitting and evaluation visit: Panel/Section Description Reason for Visit Displays previously documented reason for visit and HPI information for the encounter; this information can be reviewed and updated; this is the same panel as on other templates. Visual Acuity Displays previously documented exam findings; a new exam can be documented; this is the same panel as on other templates. General Information Used to document specific patient information related to wearing contact lenses. Contact Lens History section Displays historical information for up to two prescriptions; each prescription is displayed in separate grids for each eye. Current Contact Lens Prescription section Used for the prescription of the contact lenses the patient is currently wearing and to evaluate how the patient is doing with the current prescription. Document a Contact Lens Fitting and Evaluation Visit

Depending on the practice, workflow on the Contact Lens Evaluation - OPH template may begin after intake or after preliminary exams performed by a technician. Any information previously documented during the encounter appears in the corresponding data fields on the Contact Lens Evaluation - OPH template. This information can be reviewed and updated as appropriate. In the Reason for Visit panel, the provider reviews the technician's documentation, makes updates, and signs-off on the HPI information. In the Visual Acuity panel, enter exam findings or review and update previously documented results. visual acuity panel In the General Information panel, enter or review and update the patient information related to wearing contact lenses. general information panel In the Contact Lens History section, review and select the current contact lens prescription from the grids. contact lens history panel The prescription values appear in the fields of the Current Contact Lens Prescription section.

Access the Contact Lens Evaluation Template

Do one of the following: On the Prelim Exam - OPH template, click the Contact Evaluation link. On the Provider - OPH template, click the Contact Evaluation link. At the bottom of the Prelim Exam - OPH template in the Additional Navigation section, click the Contact Lens Evaluation link. At the bottom of the Provider - OPH template in the Additional Navigation section, click the Contact Lens Evaluation link. From the left navigation pane, click the CL Evaluation link. Document Patient Consent for Receiving Contact Lens Prescription

Before you begin To document patient consent, you must enable the Eyeglasses and/or Contact Lens Consent section on the Eye Configuration practice template. You can document the patient's consent to receive a paper or digital copy of their contact lens prescription. Then, you can generate a consent document to obtain the patient's signature. On the Contact Lens Evaluation - OPH template, in the Contact Lens/Eyeglasses Consent section, expand the Contact Lens Consent/Prescription panel. For more information on enabling the Contact Lens/Eyeglasses Consent section, go to NextGen Healthcare Success Community and download the latest Setup Guide for NextGen® Ophthalmic Suite. If the section is already enabled, go to Step 2. To indicate the patient's preference to receive their contact lens prescription, select Paper Copy or Digital Copy. If you select Paper Copy, the Patient refuses to sign that they have received the contact lens prescription check box appears next to the Print RX 1 and Pri

Record the Patient's Refusal to Sign for Contact Lens Prescription

If the patient refuses to sign for the printed copy of their contact lens prescription, you can record that in the Contact Lens Consent section. On the Contact Lens Evaluation - OPH template, in the Contact Lens Consent section, expand the Contact Lens Consent/Prescription panel. For more information on enabling the Contact Lens Consent section, go to NextGen Healthcare Success Community and download the latest Setup Guide for NextGen® Ophthalmic Suite. If the section is already enabled, go to Step 2. Select Paper Copy. If you select Paper Copy, the Patient refuses to sign that they have received the contact lens prescription check box appears next to the Print RX 1 and Print RX 2 buttons.
