Access the Contact Lens Evaluation Template
Do one of the following:
- On the Prelim Exam - OPH template, click the Contact Evaluation link.
- On the Provider - OPH template, click the Contact Evaluation link.
- At the bottom of the Prelim Exam - OPH template in the Additional Navigation section, click the Contact Lens Evaluation link.
- At the bottom of the Provider - OPH template in the Additional Navigation section, click the Contact Lens Evaluation link.
- From the left navigation pane, click the CL Evaluation link.
The Contact Lens Evaluation - OPH template opens.
