Generate Ophthalmic Referral Letters

From the Referral Letter Generator – OPH template, you can generate a referral letter to send to the patient’s primary care provider, referring provider, or other specialists. You can generate three different referral letters as needed. Click the Document Library link and then click the Referral letter generator link.The Referral Letter Generator – OPH template opens. referral letter generator - oph The read-only fields display the patient’s providers documented in the NextGen® Enterprise PM. In the Introduction section, select one of the following options to generate introductory text. Initial visit Follow up visit Opening text 1 Free text To include History of Present Illness (HPI) information in the letter, select the desired rows in the Include HPIs grid. For the following sections, select from the check boxes above the grid to indicate the information to include in the referral letter. Generate Standard Ophthalmic Letter

You can generate two different standard letters on the Document Library - OPH template. You can open the Document Library - OPH template in the following areas: Select the Documents link in the left side navigation. Click the Document Library sub-navigational link. Select one of the following: Letter 1 Letter 2 Click Letter recipients and CC to select the recipients for the letter. Select one of the following letters: Standard initial visit letter Standard follow up letter Standard diabetic exam letter Generic letter to patient The system generates the selected letter. Select Letter Recipients

You can select one or more providers to receive the referral letter on the Patient Providers - OPH template. You can access the Patient Providers – OPH template in the following locations: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. Select one of the Address letter to options to indicate the primary recipient of the letter. For example, select Address letter to P.C.P. to send the letter to the primary care provider. Note: The options are used to indicate the primary recipient of the letter. Select one or more check boxes to indicate the carbon copy recipients of the letter. Update Ophthalmology Patient Providers

To access the Patient Providers – OPH template, do one of the following: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. To update the P.C.P. field, do the following: Click the field associated with the P.C.P. to open the Modify Patient Information window. Click the arrow by the Primary Care Provider field and select a value from the list. Click OK. For the following provider fields, click the field and select a name from the list: Referring Provider Specialist CC Provider 1 CC Provider 2 CC Provider 3 Click Save & Close. Sub-navigational links are conveniently located below the template tab and you can configure them to open frequently used templates from the main tabbed templates. You can configure the sub-navigational links on the Framework Content practice template. Document an Ophthalmic Postoperative Exam

Depending on the practice, documenting a post-op visit on the Postoperative Exam - OPH template may begin after Intake or after preliminary exams performed by a technician. Information previously documented for the encounter appears in the corresponding data fields on the Postoperative Exam - OPH template. This information can be reviewed and updated as appropriate. Review and update the procedure information in the General Information section at the top of the template. Document the following details for each procedure, as appropriate: Read-only date fields across the top display the date each exam was most recently performed and documented including, Dilated exam, Gonioscopy, OCT/HRT for optic nerve, and Visual Fields. postoperative exam - oph template Enter the Reason for Visit. Enter the Visual Acuity and click Save. Record the (Intraocular Pressure)IOP and click Save. Document the External exam. Document the Slit Lamp exam.
