Document Display Settings

The Document Configuration template enables you to generate a document in run time based on preferred settings. You can configure the document elements at the provider and practice levels, but not the document format. The practice template provides the ability to: Set up general, practice-level options and how information displays on documents for: Orthopedic encounters Vital signs Behavioral Health encounters Set up provider-level content for these document types: Chart Note Consult Note Referral Letter When Chart Note is selected as the default document to print from a patient encounter, the master document is set to generate offline. The practice decides the type of documents to be routed to the Provider Approval Queue (PAQ) for sign off. Select Letter Recipients

You can select one or more providers to receive the referral letter on the Patient Providers - OPH template. You can access the Patient Providers – OPH template in the following locations: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. Select one of the Address letter to options to indicate the primary recipient of the letter. For example, select Address letter to P.C.P. to send the letter to the primary care provider. Note: The options are used to indicate the primary recipient of the letter. Select one or more check boxes to indicate the carbon copy recipients of the letter. Patient's General Information

The General panel, on the Intake template displays if the encounter is for a new or established patient. The Established patient option is selected by default if the patient has previous encounters. You can document the person providing the patient information in the Historian field. When someone else interprets the patient’s communication, you can use the Interpreter Details link to document the details for Interpreter Details category. You can use the Configurable Questionnaires link to document the details for categories available in the Selected Category picklist. The Transitioning into Care link opens the Transitioning Into Care template that captures electronic Summary of Care requests and/or receipts that are completed electronically and document the information as reportable fields on the template. Add a Provider to the CC list

You can add or update the PCP, Referring or shared provider in the CC list. Click the field for PCP, Referring or CC Provider #. Providers this Encounter- to include the PCP and/or Referring provider in the CC list or update their details. Providers to Copy on Today’s Chart Note- to include shared providers in the CC list and add or update their details. Patient Providers template The Patient Providers template opens. Select the check box for a provider who needs to receive copies to select provider names. Enter the details for the provider in the fields. Update Ophthalmology Patient Providers

To access the Patient Providers – OPH template, do one of the following: From the Document Library - OPH template, click the Letter recipients and CC link. From the Referral Letter Generator - Oph template, click the Referring Providers link. The Patient Providers - OPH template opens. To update the P.C.P. field, do the following: Click the field associated with the P.C.P. to open the Modify Patient Information window. Click the arrow by the Primary Care Provider field and select a value from the list. Click OK. For the following provider fields, click the field and select a name from the list: Referring Provider Specialist CC Provider 1 CC Provider 2 CC Provider 3 Click Save & Close. Add and Send Encounter Notes

Open NextGen® Enterprise PM. On the main toolbar, select Chart, and then in Patient Lookup, enter your search criteria, and then select Find. Matching people and patient records appear in the search results. In Matching Records, select a patient record , and then select Open. The Patient Information tab opens in the Patient Chart window. Select the Encounters tab, select the Notes tab, and then select an encounter. All notes associated with the selected encounter appear. Select the open menu icon next to Encounter Notes, select New, and then in Patient Notes, enter the necessary information, and then select OK. Note: You can select the Seal Note checkbox to lock the note information. The pushpin icon appears next to the encounter representing that the note is added. To send an encounter note, in Encounter Notes, right-click a note, select Attach to Mail, enter the recipient and other necessary information, and then select Send Message. Note: You can print, fax, and delete the note info
