Open a patient record. Click Templates on the History toolbar. The Select Template window appears. Select the Medical Records tab. In the Show section, select the All option to display the full list of templates. Select Home Page - OPH and click OK. In the General panel, select New patient or Established patient. The patient type is required for billing purposes and determines the type of visit code calculated on the Finalize tab. If the patient has been seen previously, the system automatically selects Established patient. Click the Historian field and select a value from the list or select the top blank line to type a different value. To enter a different date for First seen, click the field and select a date using the Calendar. For Vision Correction, select Yes or No. If yes, enter the Type and Years for the following: Glasses Contact Lenses Select OD or OS for Dominant Eye. Technician or assistant workflow for a patient encounter may begin on the Home Page - OPH template when the patient first arrives. You have the option to start or continue the Technician or Assistant workflow on the Intake – OPH template. Access Home Page - OPH How to access the Ophthalmology Home Page template. The History Summary panel on the Intake-OPH template enables you to document a patient's past medical or surgical history, interim, social, family, diagnostic study, and glucose monitoring history. It also displays the patient history documented by the Intake staff on the Histories template. You can also add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by clicking the Add to HPI Grid button. The grid includes a list of categories in the left pane to select and view the documented data.
