Add a New Assessment on the Common Assessments Template

On the Assessment/Plan - OPH template, click the Common Assessment link. The Common assessment template opens displaying common assessments for Ophthalmology. The template displays common assessments for Retina for a provider configured as a Retina specialist on the Eye Configuration practice template. For more information, go to NextGen Healthcare Success Community and download the latest Setup Guide for NextGen® Ophthalmic Suite. common assessment window To change to a different ophthalmic specialty, click the Common assessments field and select one of the following: OPH Retina Assessment RET - Retina Ophthalmology OPH Assessment EYE - General Ophthalmology Click an assessment listed with a code to add it to the Today’s Assessments grid. Assessments listed without a code are assessment categories. Clicking an assessment category opens a list of assessments. Select an assessment from the list and then click OK. Links Found in the Plan Section of Assessment/Plan - OPH

The Plan section has the following links for documenting a plan. Link Description Cataract Post-Op Complications Opens the Cataract Post-Op Complications template used to document patient complications after cataract surgery. Quick Plan Opens the Quick Plan feature used to quickly select a predefined plan based on condition. OPH Plan Details Opens the OPH Plan Details template used to document an Ophthalmology specific plan. Order Sets Opens the core My Plan or My Custom Plan template and the My Plan template set. This template set is not specific to Ophthalmology. My Phrases Opens the My Phrases feature used to document details using a provider's saved frequently used phrases. Order Sets

On the Assessment/Plan – OPH template, the Order Sets link in the Plan section opens the My Plan tab of the My Plan/Orders tabbed template set. The My Plan template is a template where orders are placed based on client customizable order sets. Order sets are based on disease categories that correspond to the selected assessment. When you highlight an assessment in the upper left of the My Plan template, the template displays the orders for the disease category associated with the selected assessment. my plan template There are four order sets for Ophthalmology: diabetic retinopathy glaucoma cataracts macular degeneration Diagnostic tests can be set up by clients to be listed in either the Diagnostics or the Office Procedure sections of the My Plan template. Note: Orders in the Diagnostics section are sent to the Order module. Orders in the Office Procedures section do not go to the Orders module. You can set up diagnostic tests/studies such as X-ray and CT scan in these sections of the

Document a Plan Using Quick Plan

The easiest way to document an Ophthalmology plan is to use the Quick Plan feature. The Quick Plan link opens a list of plans for common ocular conditions. These plans can be configured at the practice level or they can be edited at the individual patient level. Click the Quick Plan link in the Plan section of the Assessment/Plan – OPH template. A list of quick plans available in the system for Ophthalmology opens. oph quick plan template Click a plan in the list. Place Orders with My Plan

On the My Plan template, highlight an assessment in the upper left of the template in the Today's Assessments grid. Select the check boxes corresponding to items to be ordered. To select an order item not displayed: Click the down-arrow of an available field in the appropriate order type section to open a list of all items available in the system for that order type. Locate the item you want to order in the list, highlight it and click OK. The selected item appears in the field. Select the check box corresponding to the item to order it. Click Place Order.When you place orders for labs and radiology diagnostic studies, you or someone from your office must process and submit them to the Order module. Click the Lab/Diagnostics/Radiology Order Module Processing link found in the lower right corner of the template to launch the Process and Submit Lab/Radiology Orders template. Document a Detailed Ophthalmic Plan

You can document a detailed plan on the Oph - Plan Details template. On the Assessment/Plan - OPH template, in the Plan section, the OPH Plan link opens the Oph - Plan Details template. As you select details from different sections of the template, a summary statement appears in the Plan summary field. oph - plan details template Click the OPH Plan link to open the Oph - Plan Details template. Select the appropriate eye from the following: OD OS OU Select the appropriate check boxes in the following sections to document a detailed plan: General Discussions Medication Glasses/Contact Lenses Surgery Procedures Patient Instructions Tests Completed Today IOP Control In the Education Material section, select Education material provided and select the materials discussed with the patient.
