Confirm Changes

Once you have decided which actions to perform you must confirm your changes for each clinical data type (Medications, Medication Allergies, and so on). Once you confirm your changes, the Clinical Reconciliation module updates the patient's chart. It does not update the information in the Import section or the imported document. Reconcile clinical data. Resolve Missing or Incorrect Data. Resolve duplicate items. Resolve medication contraindications or drug interactions. Acknowledge medication deletions. Perform One-Click Reconciliation

After you have clicked on or hovered over the reconcile button in the heading for a section, the Reconcile All New <Items> window opens. The New <Items> pane shows the items that are available for import. Initially, the action for each item is set to Add, which means the item will be imported. Note: The titles of the window and the individual panes vary to reflect which C-CDA section you are reconciling. For example, if you are reconciling the Allergies section, the window title is Reconcile All New Allergies, and the upper right pane is named New Allergies. Actions for New Items To change the action for a new item, select the value in the Action column, and selectAdd or Ignore. Selecting the Action for a New Item After you have finalized your selection, select Confirm to import the selected items. The items that you removed from the selection are marked as ignored. Reconcile Clinical Data

The Import tab enables you to review clinical data stored in a patient's chart and reconcile the data with contents of an imported document or of a patient medication history file. Note: The following procedure shows the basic steps for performing reconciliation. Detailed procedures appear later in this section. Open the Clinical Reconciliation module from NextGen® Enterprise EHR, the Data Share module, or the Medical Summary Utility and then select a document. Select the Import tab. For an overview of the screen layout, refer to Layout of the Import Tab. Under the Import tab, there is a second set of clinical data tabs. The reconcile a particular type of clinical data, select the corresponding tab, such as Medications, Meds Administered, and so on. If you opened Clinical Reconciliation from a specific module, such as Medications or Problems, then Clinical Reconciliation displays only the tab corresponding to that module. Process Items in the EHR Section

The EHR section displays clinical items from the patient's chart based on the Import tab selected (Medications, Medication Allergies, and so on). For each item listed you can choose to perform an action. Select the type of clinical data you want to reconcile. Process items in the Import section. Select the item you want to reconcile. Items to Reconcile Select an item in the Action column. Selecting an Action Select one of the following actions action (if any) to perform: Keep - No action is taken. Stop - Stop a medication. Delete - Delete the item from the patient's chart. Resolve - Resolve a medication allergy or problem. By default, all clinical items in the EHR section have a default action of Keep. Note: You cannot delete a clinical item in a locked encounter or added by another practice in the enterprise (namely, enterprise chart). Match/Unmatch clinical items as needed. Process Items in the Import Section

The Import section displays the clinical data available for import based on the Import tab selected (Medications, Medication Allergies, and so on). For each item listed you can choose to perform an action. All items in the Import section have a default action of Add if they do not exist or match to an item in the patient’s chart. If the Clinical Reconciliation module determines that an item in both the EHR and Import sections match, it sets the Import action to Ignore automatically. Select the type of clinical data you want to reconcile. Select the item you want to reconcile. Items to Reconcile To view additional clinical information, slide the scroll bar at the bottom of the Import section right. Select a column header to sort data in ascending/descending order. If you are on the Medications tab or the Medications Administered tab, you can right-click the Import section and select Apply Default Sort to display the original view. Resolve Missing or Incorrect Data

If a clinical item that you selected for import has missing or incorrect data, a search window appears and, if available, displays a list of potential matches. If the item does not appear on the list or the application cannot find any potential matches, you can perform a search. Search for Matches Perform a search: Select Clear. Enter a full or partial description. If the entry is fewer than five letters, select the Enter key. If the entry is five letters or more, the application performs the search automatically. The application displays a list of potential matches. For Medications and Meds Administered, you can select a Medication Search Type (all, brand name, or generic) to filter results. Do one of the following: To select an item and continue processing, select an item in the grid and then select OK. To select another item or perform another search, select Cancel.
