Document a Patient's Height, Weight, BSA, Temperature, Blood Pressure and Pulse

You can document a patient's height, weight, BSA, temperature, blood pressure and pulse using the Vital Signs panel. The Vital Signs panel is where you can view vital signs documented during this encounter. Previously entered vital signs can be edited or removed, and new vital signs can be entered. The Vital Signs panel is also found on other office visit templates. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template.The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link.The Care Guidelines grid view template opens. In the Vital Signs panel, select Add. Vital Signs The Adult Vital Signs [New Record] template opens. The Measured Date, Time and the Measured by fields fill in automatically. Note: You can change the date but you cannot modify the user name. Documenting Vital Signs

You can document and update a patient's vital signs on the Vital Signs template. You can edit or remove a vital signs record from the Vital Signs grid. The vital signs template names vary based on a patient's age. For an adult patient, the template name is Adult Vital Signs. If a patient is younger than two years, the template name is Pediatric Under 2 Years Vital Signs. If a patient is two years or older, the template name is Pediatric Over 2 Years Vital Signs. You can change a pediatric patient's age to an adult age on the Practice Configuration template. A patient's height and weight fields are used to calculate their Blood Mass Index (BMI). BMI is not available for patients who are younger than two years. You can configure the normal vital signs range. If a patient's vital signs are outside the normal range, a message appears. Document Vital Signs

You can document the vital signs of a patient from the Vital Signs panel of the Intake - Cardiology template. The vital signs template is based on a patient's age. For an adult patient, the template is Adult Vital Signs. For a patient who is younger than two years of age, the template is Pediatric Under 2 Years Vital Signs. For a patient who is two years or older, the template is Pediatric Over 2 Years Vital Signs. Open NextGen® Enterprise EHR. Open the Intake - Cardiology template and click the Vital Signs panel. Click Add. The vital signs template opens depending on the age of the patient. Enter the following details on the vital signs template. Height/length measurements Weight measurement Temperature Blood Pressure and pulse Respiration and Pulse Ox Peak flow Pain level BMI/BSA calculation For a female patient, enter the Last Menstrual Period (LMP) details. Access the Vital Signs Template

You can access the vital signs template to document the patient vital signs. Based on the configuration of the Pediatric Age section on the Practice Configuration template, different vital signs templates appear for adult patients, pediatric patients younger than two years, and pediatric patients older than two years. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add. The age-specific vital signs template opens. Document the Patient's Height You can document the height of a patient in inches or in feet using the Vital Signs template. Enter Vital Signs

Use the Vital Signs link to document vital signs obtained during the visit. Select the Vital Signs link. The Vital Signs template opens. Select the Add button on the Vital Signs template. The Vital Signs (Adult or Pediatric) template opens. Vital Signs template Document vital signs information such as Height, Weight, BP and Pulse. Select Save. Select Close to close the template. Record the Pre-Procedure Vital Signs of the Patient

In the Vital Signs Monitoring panel, click Add. The Vital Sign Monitoring template opens. The Baseline Vital Signs section includes a grid to display only the baseline set of vitals documented in the Vital Signs Monitoring section in the template. In the Vital Signs Monitoring section, enter the required details. For more details, see the Enter the Vital Signs Monitoring Details. Click Save & Close to display the documented vital signs in the grid. The baseline vitals display at the top row of the grid throughout the workflow. The Height, Weight, BMI, and BSA above the grid displays the height, weight, and the calculated BMI and BSA values from the Vital Sign Monitoring template. The values associated with the Height, Weight, BMI and BSA labels above the grid dynamically update as you modify or add height and weight data on the Vital Sign Monitoring template.
