Document Vital Signs

You can document the vital signs of a patient from the Vital Signs panel of the Intake - Cardiology template.

The vital signs template is based on a patient's age. For an adult patient, the template is Adult Vital Signs. For a patient who is younger than two years of age, the template is Pediatric Under 2 Years Vital Signs. For a patient who is two years or older, the template is Pediatric Over 2 Years Vital Signs.
  1. Open NextGen® Enterprise EHR.
  2. Open the Intake - Cardiology template and click the Vital Signs panel.
  3. Click Add.
    The vital signs template opens depending on the age of the patient.
  4. Enter the following details on the vital signs template.
    • Height/length measurements
    • Weight measurement
    • Temperature
    • Blood Pressure and pulse
    • Respiration and Pulse Ox
    • Peak flow
    • Pain level
    • BMI/BSA calculation
  5. For a female patient, enter the Last Menstrual Period (LMP) details.
  6. To indicate that certain vital signs are unobtainable, select the Unobtainable check box, and select an option from the list that appears.
    The corresponding field gets disabled.
  7. To indicate that the patient refused to provide certain vital signs, select the Patient Refused check box, and select an option from the list that appears.
    The corresponding field gets disabled.
  8. Enter your comments in the Comments section.
  9. Click Save and then click Close.
