Place a Follow-up/Disposition Order

Do one of the following: In the Assessment Plan panel, click the A/P Details link in the left pane. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Click Follow Up. The Follow-up/Disposition template opens. Follow-up/Disposition template To add or update the working assessment, select the diagnosis in the grid. Select the follow up check box. Enter the details and time for the follow-up visit. In the Disposition field, select an option from the list. If the applicable option is not available in the list, click the top blank row and then type a value in the Disposition field. If the patient refused the selected disposition, select the Refused check box. In the Reason field, select an option from the list. In the Timeframe field, select an option from the list. Click Place Order. The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the Assessment/Plan grid. Click OK. Parent topic:

Enter Additional A/P Details

You can document additional information such as a differential diagnosis, plan details, or comments for the assessment on the Assessment/Plan Details template. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Assessment Plan Details To add or update the working assessment, select an assessment in the Today's Assessments grid. Type or dictate patient data in the Impression/Comments, Plan Details, Differential Diagnosis, and Provider Details. For a follow-up visit or disposition is needed, place the order. Shared Working Assessment

The group of templates that share the selected assessment for the patient. You can initially select or change the working assessment in any of the templates that share the working assessment. If a single assessment exists, the application selects it as the working assessment. If you remove one of multiple assessments on a template, you must select a new working assessment. On some templates, the working assessment appears in the Selected Diagnosis field, Selected Dx field, or Selected Assessment field. On other templates, the working assessment appears in the Today's Assessment grid. The last assessment or diagnosis you select on any of the following templates carries over as the working assessment when you open another template in the group. Place Orders from the My Plan Template

You can place orders for the patient using the My Plan template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link. The Care Guidelines grid view template opens. Expand the Recent Orders panel and select Add. The My Plan template opens. My Plan The My Plan template displays order items in the set appropriate for the selected assessment’s diagnosis category. To use a different template in the orders set, select the tab corresponding to the template to use (for example, Labs or Diagnostics). To add or change a diagnosis in the Today's Assessments grid, select Add or Edit. To change the current working assessment, select the diagnosis addressed for this encounter in the Today's Assessments grid. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the

Using the Assessments Tab

The Assessments tab in the My Plan template set houses the Add or Update Assessment template. The grids on the Assessments tab can be filtered. For example, in the Billing Diagnosis History section, selecting the Show Chronic only check box displays only those diagnoses that are not acute. Selecting the option to Add Assessments on 1 click speeds up the documentation process by including the diagnosis description and code for Today's Assessment in the grid and Problems module without having to click Add/Update. The Medications or Immunizations links can be used at any time during the encounter to access the associated module.
