Place a Follow-up/Disposition Order

Do one of the following: In the Assessment Plan panel, click the A/P Details link in the left pane. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Click Follow Up. The Follow-up/Disposition template opens. Follow-up/Disposition template To add or update the working assessment, select the diagnosis in the grid. Select the follow up check box. Enter the details and time for the follow-up visit. In the Disposition field, select an option from the list. If the applicable option is not available in the list, click the top blank row and then type a value in the Disposition field. If the patient refused the selected disposition, select the Refused check box. In the Reason field, select an option from the list. In the Timeframe field, select an option from the list. Click Place Order. The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the Assessment/Plan grid. Click OK. Parent topic:

Document the Treatment Plan on My Plan

The treatment plan for the patient can be documented on the My Plan (PT) template that enables providers to place multiple orders from a single template. The My Plan (PT) template is based on order sets for diagnosis categories that can be modified and managed by clients using practice templates. The My Plan (PT) template appears within NextGen® Adaptive Content Engine by default and can be easily replaced with the My Custom Plan template after a simple configuration in the Practice Configuration practice template. The My Custom Plan template can be personalized to suit your practice's specific needs and is less click-intensive. Assessment/Plan Panel

The Assessment/Plan panel provides a summary of the assessments and plan information for the patient for the current encounter. In Physical Therapy, assessments for the encounter are selected at the beginning of the encounter, therefore the Assessments/Plan panel grid displays the previously selected assessment(s). The A/P Details link in the left side navigation opens the Assessment/Plan Details and Treatment template. Use this template to add as many impressions, comments, differential diagnoses, and patient and provider’s plans for a diagnosis, as necessary. Enter Assessment Plan Details

Open NextGen® Enterprise EHR. On the Assessment Plan Details template, select the A/P Details tab. To add or change a diagnosis in the Today’s Assessments grid, select Add or Edit to open the Add or Update Assessment template. To change the current working assessment, in the Today's Assessments grid, select the diagnosis being addressed for this encounter. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the assessment. Enter details manually or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details link.
