Place a Follow-up/Disposition Order
Do one of the following:
In the
Assessment Plan
panel, click the
A/P Details
link in the left pane.
On the
My Plan (PT)
template, click the
A/P Details
tab.
The
Assessment Plan Details
template opens.
Click
Follow Up
.
The
Follow-up/Disposition
template opens.
To add or update the
working assessment
, select the diagnosis in the grid.
Select the
follow up
check box.
Enter the details and time for the follow-up visit.
In the
Disposition
field, select an option from the list.
If the applicable option is not available in the list, click the top blank row and then type a value in the
Disposition
field.
If the patient refused the selected disposition, select the
Refused
check box.
In the
Reason
field, select an option from the list.
In the
Timeframe
field, select an option from the list.
Click
Place Order
.
The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the
Assessment/Plan
grid.
Click
OK
.

Shared Working Assessment
The last assessment or diagnosis you select on any of the following templates carries over as the working assessment when you open another template in the group: Add or Update Assessment Assessment Plan Details Diagnostic Studies Chemistry Lab Hematology Lab Master Microbiology Other Labs Add or Update Assessment Therapeutic Levels Serology Tumor markers Add or Update Assessment Urine Chemistry Office Procedures My Plan Note: The My Custom Plan template and My Plan (PT) template are not part of the template group that shares the working assessment. Enter Additional A/P Details
You can document additional information such as a differential diagnosis, plan details, or comments for the assessment on the Assessment/Plan Details template. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Assessment Plan Details To add or update the working assessment, select an assessment in the Today's Assessments grid. Type or dictate patient data in the Impression/Comments, Plan Details, Differential Diagnosis, and Provider Details. For a follow-up visit or disposition is needed, place the order. Document the Disposition for a Follow-Up
In the A/P Details tab, click Follow Up. A/P Details The Follow-up/Disposition template opens. In the Disposition section, do one of the following: Click the Disposition field to select an option from the list. Click the top blank row to type a value in the Disposition field. To indicate the patient does not want to comply, select Refused. To select an explanation for the disposition, do one of the following: Click the Reason field to select a value. Click the top blank row to type a value in the Reason field. To indicate a recommended time for the follow-up disposition, do one of the following: Click in the first Timeframe field to select a preset value or click the top row in the list to type your own timeframe. Click in the second Timeframe field to select a month or click the top row in the list to type your own value. Click Place Order. On the Assessment Plan Details template, in the A/P Details tab, click Follow Up. Assessment Plan Details template The Follow-up/Disposition template opens. Follow-up/Disposition template Document an assessment. In the Follow-up section, select the follow up check box. Click the with field, select a provider's name from the list, and click OK. Enter the time or date limit and timeframe for the follow-up visit. To select a follow-up reason, click the Reason field, and then do one of the following: Select a preset option from Reason for follow-up visit list. To enter a custom reason, click the blank row at the top of the Reason for follow-up visit list, and then type the text in the Reason field. The template displays a statement at the top of the Follow-upsection summarizing your entries for the provider, time limit, date limit, time frame, and reason for the follow-up visit. In the Orders panel, click Follow Up. follow-up/disposition The Follow-up/Disposition template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates in the group as the working assessment. The system defaults to the current working assessment. If required, you can select another assessment, which becomes the new working assessment that carries forward to the other templates in the group. Do one of the following: If not already selected, select the diagnosis in the grid and then select the follow up check box along with other details and the suggested time for the follow-up visit. Click Add/Edit Assessment to enter a new diagnosis or modify an existing assessment for the current patient. With the correct description selected in the grid, in the Follow-up section, select follow up along with any other details and the suggested time for the follow-up visit. In the Assessment / Plan panel, select the Follow Up button. The Follow-up/Disposition template opens.
