Document Follow-Up Details

  1. On the Assessment Plan Details template, in the A/P Details tab, click Follow Up.

    The Follow-up/Disposition template opens. Follow-up/Disposition template
  2. Document an assessment.
  3. In the Follow-up section, select the follow up check box.
  4. Click the with field, select a provider's name from the list, and click OK.
  5. Enter the time or date limit and timeframe for the follow-up visit.
  6. To select a follow-up reason, click the Reason field, and then do one of the following:
    • Select a preset option from Reason for follow-up visit list.
    • To enter a custom reason, click the blank row at the top of the Reason for follow-up visit list, and then type the text in the Reason field.
    The template displays a statement at the top of the Follow-upsection summarizing your entries for the provider, time limit, date limit, time frame, and reason for the follow-up visit.
  7. To enter the phone number for the follow-up appointment, click the Phone number field and select the number.
  8. Click Place Order.
  9. Click OK.
    If you click OK without placing an order, a confirmation message appears.
    • To place the order, click Yes.
    • To close the Follow-up/Dispositiontemplate without placing the order, click No.
