Document Follow-Up Details
- On the Assessment Plan Details template, in the A/P Details tab, click Follow Up.
The Follow-up/Disposition template opens.
- Document an assessment.
- In the Follow-up section, select the follow up check box.
- Click the with field, select a provider's name from the list, and click OK.
- Enter the time or date limit and timeframe for the follow-up visit.
- To select a follow-up reason, click the Reason field, and then do one of the following:
- Select a preset option from Reason for follow-up visit list.
- To enter a custom reason, click the blank row at the top of the Reason for follow-up visit list, and then type the text in the Reason field.
The template displays a statement at the top of the Follow-upsection summarizing your entries for the provider, time limit, date limit, time frame, and reason for the follow-up visit. - To enter the phone number for the follow-up appointment, click the Phone number field and select the number.
- Click Place Order.
Note: Place Order is active only after you select the diagnosis and the follow up check box.
- Click OK.
If you click OK without placing an order, a confirmation message appears.
- To place the order, click Yes.
- To close the Follow-up/Dispositiontemplate without placing the order, click No.
- Enter the Time Limit and Timeframe for the Follow-up Visit
You can set up the time period for a follow-up visit on the Follow-up/Disposition template.
