Add Details for a Treating Diagnosis

Enter Additional Assessment and Plan Details

You can document additional information such as a differential diagnosis, plan details, or comments for the assessment on the Assessment/Plan Details template. On the My Plan (PT) template, click the A/P Details tab.The Assessment Plan Details template opens. Assessment Plan Details To add or update the working assessment, select an assessment in the Today's Assessments grid. Type or dictate patient data in the Impression/Comments, Plan Details, Differential Diagnosis, and Provider Details. For a follow-up visit or disposition is needed, place the order. Open the Assessment Plan Details Template

Some specialties (for example, Cardiology) open the Assessment Plan Details template differently. Refer to the specialty guide for your version of NextGen® Adaptive Content Engine available on NextGen Healthcare Success Community for instructions if the following does not apply: Open NextGen® Enterprise EHR. On the SOAP template, in Assessment/Plan, select A/P Details.The Assessment Plan Details template opens. Assessment or Plan Panel

The Assessment/Plan panel provides a summary of the assessments and plan information for the patient for the current encounter. In Physical Therapy, assessments for the encounter are selected at the beginning of the encounter, therefore the Assessments/Plan panel grid displays the previously selected assessment(s). Document Information in the Assessment and Plan Panel

You can document information in the Assessment and Plan panel. The Assessment/Plan panel allows you to enter an assessment, generate the prenatal record, enter a follow-up appointment if not already done on the OB flowsheet, document which hospital the prenatal record must be sent to, and task to send the prenatal record to the hospital. The Prenatal Record button generates the cumulative Obstetric document for the pregnancy to date. At the first visit, it is important to generate the Prenatal Record to review all of the information for completeness and accuracy. After reviewing the Prenatal Record, the provider may note data that was not entered or view information that must be entered in the Problem/Detail list for easy viewing at subsequent visits.
