Reviewing Patient History on SOAP

You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Past Medical/Surgical Interim Social Family Diagnostic History Panel The Social Determinants of Health and PRAPARE® links appear based on the Practice Configuration practice template settings. Review Patient's History Summary on SOAP You can review and update a patient's history summary documented by the intake staff during the previous encounters, in the History panel on the SOAP template. Adding Patient's History Summary on SOAP You can update the history details entered by the intake staff and add any additional history details in the History panel on the SOAP template. Patient Tracking in the Provider Workflow

You can update the patient tracking details using the Patient Tracking on the Patient Information Card (PIC). You can select the Patient Tracking button on the Patient Information Card (PIC) and update the tracking details on the Todays Patient Tracking template. Patient Tracking is available only when the patient has an active encounter dated Today. For all other encounters, the Patient Tracking button appears as New Encounter. You can hover over the Patient Tracking button to view a quick summary of the patient tracking records for the visit on the PIC. Before starting the patient assessment on the SOAP template, you can indicate that the patient is with the provider by changing the status to with provider in the Todays Patient Tracking template. When the patient assessment is completed, you can change the status to ready for check-out. Associate Medications

The Associate Medications template enables you to view and associate an active medication with up to three active diagnoses. You can use the Associate Medications template to associate medications and diagnoses instead of navigating back and forth from the Medications module to the Chronic Conditions template to identify and associate all of the active medications for the problem. The Diagnosis, Medication, ICD Code, Dx1, Dx2, and Dx3 fields are filled only if you have selected a problem and the associated medication in the Chronic Conditions template. The Active medications grid displays all the active medications of the patient and the Active diagnoses grid of the patient displays all the active diagnoses associated with the patient. The information icon provides information about the Associate Medications template. Review of Intake Details by Provider

You can start the visit by reviewing the patient's details entered by the intake staff on the Home Page template, to determine if any updates should be made. The Home Page template consists of the following panels. Panel Description Medical Chart Summary Displays the details documented by the intake staff and provider during an encounter. The details from the previous encounters also appear in this panel. The categories in the left pane in the grid enable you to select and view the documented data. Some of the categories include filters at the top of the grid, which enable you to filter and review the desired data. The data that appears in the grid is read-only. Problem List Displays the patient's chronic and secondary problems documented by the intake staff. History Summary Displays the patient history details previously document by the intake staff and provider on the Intake, Histories, and SOAP templates. Reviewing Patient's General Information on SOAP

You can view the general details of a patient in the General panel on the SOAP template. In the General panel, you can do the following: Document the relationship of the person who provides the patient information, in the Historian field. Capture the electronic Summary of Care requests and receipts that are completed electronically, and provide the information as reportable fields on the Transitioning Into Care template. On the Addition Visit Information template, you can provide answers for the preconfigured questions in the Interpreter Details category. You must document interpreter details when someone else interprets the patient’s communication for you. Indicate if the patient for the current encounter is new or established. SOAP Template

You can review and modify the documented patient intake and histories information on the SOAP template. You can review the Asthma Flowsheet, Allergy Test Flowsheet, Spirometry, and Nebulizer panels. You also can document physical exam, assessment plan, and communication details. SOAP Link Icons The icons for various links appear on the SOAP template. Document the Patient's Reason for Visit as a Provider The reason for visit added by the intake staff on the Intake template appears on the SOAP template. Patient's History Summary Documentation by Provider You can document a patient's past medical or surgical history, interim, social, family, and diagnostic study history in the History Summary panel.
