You can document the communication details on the Provider Communication template.
- Open NextGen® Enterprise EHR.
- Open the SOAP template.
- Select the Provider Comm
icon.
The Provider Communication template opens.
- The Date field displays today’s date. To select another date, select the Date field and select the date.
- Enter the reason for communication in Communication regarding.
Note: When you select
high osteoporosis fracture risk as the reason, then the diagnosis search window must open. If the diagnosis search window is not opening, then you need to change the value of

- Enter the recipient in To.
- Enter the name of the recipient in Name.
The Diagnosis and Code automatically display the previously documented assessment.
- If the communication attempt is completed, then select Communicated and enter the method in Via.
- If the communication attempt is not successful or not completed, then select Not communicated and enter a reason.
- Enter Comments.
- Select Add/Update.
The documented communication appears in the grid on the
Provider Communication template.
The Date and Time fields display when the record gets created, and the Edited by field displays the name of the user who creates the record.
- Save and close the template.
