Patient Information Card

You can view pertinent information about a patient through the Patient Information Card (PIC).

To use the Patient Information Card (PIC), the practice must first configure it in File Maintenance. The PIC provides details about the patient’s general details, PxP Portal enrollment status, alerts, tracking details, demographics, social history, telephone conversations, send tasks, encounter data and indication. You can also send and receive emails using Inbox. You can access certain modules to view patients' allergies, problems, diagnoses, medications, and so on from Top Toolbar above the PIC. The PIC includes the following links to document the details of the patients:

  • Referring Provider enables you to open the Patient Providers template, which includes primary care and encounter providers details. You can enter additional referring provider details and select providers to include on the Visit Note document. You can also copy other providers on the patient’s chart note.
  • HIPAA enables you to open the HIPAA Disclosure Information template to document the authorization provided by patients or their legal guardian to disclose protected health information. The HIPAA Information section is documented in NextGen® Enterprise PM when the patient arrives.
  • Advance Directives enables you to open the Advance Directives template, where the Directives on File section provides a quick reference to a patient’s advance directives. To review specific details, see the Advance Directive document on file.
  • Screening Summary enables you to open the Screening Summary template to document the patient screening details required by accrediting organizations. Clinical support staff uses the Counseling/Educational Factors link to record information on counseling they provide. The same link appears in the Finalize template and is often used by providers to document the counseling time spend on counseling a patient.
  • OBGYN enables you to open the OBGYN Synopsis template to document additional gynecological information. The OBGYN link appears for female patients who are eight years or older. If the patient is younger than eight years, the Birth History link appears.
