My Plan Tabbed Template Set
The following functions are available on the tabs in the My Plan template set: Assessments- addresses the needs of both ICD-10 coding and 5010 standards My Plan- can be set to use the My Plan or My Custom Plan template; My Custom Plan includes the following functionality: If an advanced imaging order meets the Appropriate Use Criteria (AUC), the system saves the associated AUC information. Hold order processing and a robust Ask at Order Entry feature A/P Details- enter any pertinent comments, differential diagnoses, plan or provider details. Labs- enter new lab orders associated to an assessment; can also be set up to bypass the template for order processing. Diagnostics- includes the ability to: Document Appropriate Use Criteria (AUC) scores and other information for advanced imaging orders to meet AUC mandates. My Plan vs. My Custom Plan to Create a Treatment Plan
If configured, the My Plan tab houses an enriched plan template called My Custom Plan, which can be used to document a treatment plan that is personalized to suit your practice's specific needs. The custom plan functionality is configured as a grid for order sets, replacing the need to manually select a check box for each order to be placed. The individualized fields available on the original My Plan require manual selection of every order needed for a particular diagnosis. The purpose for the My Custom Plan template is to reduce the number of clicks required for ordering while maintaining the process for managing order sets and provider plans. The My Custom Plan template still takes a personalized approach to diagnosis-based order sets. The custom plan template simply uses a more streamlined user interface. Configure AUC Scores
Configure AUC Scores Configure AUC Scores You must configure settings in File Maintenance and on the Practice Configuration practice template to use the AUC score wizard. To successfully use the AUC score wizard, you must configure the following settings in File Maintenance. Open NextGen® Enterprise EHR. To activate the AUC rules for the assigned payer, in the File Maintenance, do the following. In the Master Files, click Payers. The Payers List window opens. Double click a payer from the list. The Modify Payer Information window for the payer opens. Click the Orders Module tab, and select the Enable payer for Appropriate Use Criteria rules for Advanced Radiology Imaging orders check box. Apply the AUC for all payers within the same financial class, if necessary. To automate the inclusion of modifier and G-code in the procedure billing, select the Include AUC /HCPCS code modifier automatically option. Select a payer in the encounter level insurance for the current patient. In the Order

Configuring Preferences for Assessments
There are many settings configured at the practice-level on the Practice Configuration template that determine how patient assessments appear in the My Custom Plan template: Assessment Status- determines if recording the status of each diagnosis is a required element in the documentation of patient encounters Staff to Lookup Diagnosis- enables the provider to instruct support staff regarding diagnosis selection by tasking One Click Assessment- time-saving functionality that may or may not be enabled as per your practice's standards Problem List Note- determines whether the most recent note from the current user or from all users displays for the selected problem Appropriate Use Criteria (AUC)- when enabled, selecting an advanced radiology order that meets the criteria initiates related functionality and records data collected on the ACR score window Access the Practice Configuration Template Make an Assessment Status Required Hide or Show the Staff to Look up Diagnosis Link Bypassing t
