Enter and View Diagnoses on My Custom Plan
Orders can only be created after an assessment is designated for the encounter. On the My Custom Plan template, My Plan panel, do one of the following actions: Next to Diagnosis Selection, select the desired search option (Today's Assessments or Clinical Problems) to highlight the assessment in the grid. Click the Dx Search link to open the diagnosis search window. Search for and highlight the correct diagnosis. Click Select to return to My Custom Plan. Click Add/Edit Assessment or Add/Edit Problems below the grid (depending on the search option selected) to add as many assessments and/or clinical problems as is required. Click Save & Close to return to My Custom Plan. Note: There are five diagnoses or four clinical problems immediately visible in the grid with the option to scroll down to view all of the entered data. The assessments can be sorted on the Add or Update Assessment template (Assessments tab in the My Plan tabbed template set). After a new order is created, the diagnosis

My Plan vs. My Custom Plan to Create a Treatment Plan
If configured, the My Plan tab houses an enriched plan template called My Custom Plan, which can be used to document a treatment plan that is personalized to suit your practice's specific needs. The custom plan functionality is configured as a grid for order sets, replacing the need to manually select a check box for each order to be placed. The individualized fields available on the original My Plan require manual selection of every order needed for a particular diagnosis. The purpose for the My Custom Plan template is to reduce the number of clicks required for ordering while maintaining the process for managing order sets and provider plans. The My Custom Plan template still takes a personalized approach to diagnosis-based order sets. The custom plan template simply uses a more streamlined user interface. Update a Referral Associated with an Order Set
Open the My Custom Plan template. Highlight a row from the order set populating in the grid for the Pending Orders section. In the Order Options section, highlight the order to be modified and make the necessary changes to the referral. Click Update. Notice the referral is updated in the grid under Pending Orders. Order Timeframes and Where They Apply
The ability to add an approximate date above the Timeframegrid is available for the following plan types on the My Custom Plan template: Lab Orders Diagnostics Office Medications Office Procedures Office Supplies Office Labs Referrals Followup The system populates a date in the field that is equal to the current date plus the selected value for Timeframe in the grid under Pending Orders. The date field recalculates that date each time a new timeframe is selected (for example, one week or one month, and so on. My Plan In the Assessment/Plan panel, the timeframe data collected is concatenated to display as: <Time Frame text> on approximately <Date from Date field> (for example, in 1 Week on approximately 11/31/2017). My Custom Plan Functionality
When configured to display, the enriched plan template includes the following functionality on two panels: My Plan: By default, the panel is expanded when the template opens. Colored indicators appear for the status of the Ordering Assessment, Diagnosis Group, and Order Set. Manage Orders: Used to add, update and remove details (for example, side, site, position, status, appointment date, time, location, time frame, orientation and position, and update order status). Includes pre- and post-procedure directions from the Procedure/Instructions link. Access the Procedure AOE Template
On the My Custom Plan template, My Plan panel, highlight a diagnosis. Highlight the required procedure order and click Add to populate it to the Pending Orders. Click AOE to open the Procedure AOE template. My Plan Note: If you do not click AOE and procedures are available with AOE questions, upon clicking Place Order, the Procedure AOE template automatically opens.
