Entering Assessments, Impressions, Differential Diagnoses, and Associated Details

On the Assessment / Plan Details template, the provider selects an assessment and then enters any pertinent comments, differential diagnoses, plan details, provider details, or today's orders with the option to use My Phrases or Common Phrases (practice-wide phrases), and Follow-Up. Entering information in A/P Details tab is optional. The A/P Details tab includes the following sections: Today's Assessment grid: You can add, edit, sort, and remove rows. The columns include the diagnosis priority number (#), Description, Code, and Status. Impression/Comments memo field: You can enter text, record audio, and access My Phrases. The data entered here also appears in the Assessment / Plan panel. You can select Comments link to open the A/P Details tab directly from Assessment / Plan panel to update the comments. Access the Add or Update Assessment Template

Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click Add/Update. The Add or Update Assessment template opens. Open the Assessments Tab

There are several options for accessing the Add or Update Assessments template: On the SOAP template, in the Assessment / Plan panel, select Add/Update. On the Intake or SOAP templates: Select the My Plan link to open the template set. My Plan Select the Assessments tab. Upload the Procedure Diagnosis in Today's Assessment Grid

In the Documentation and/or Findings panel, select the values that are associated with the procedure diagnosis. Expand the Assessment/Plan panel and click the Auto Dx button. The Add or Update Assessment template open. The system uploads the procedure diagnosis in the Today Assessment grid. Add or Update Assessment Update the assessment details in the Today's Assessments grid. Click Save & Close. The diagnosis details appear in the grid. Upload the Billing Data in the Procedure Billing Data

In the Procedure Billing panel, click the Auto Billing button. The billing data uploads to the grid. The Auto Billing button becomes unavailable for selection. Note: To display a diagnosis code in the grid, you need to first click the Auto Dx button in the Assessment/Plan panel to upload the diagnosis details in the Today's Assessments grid on the Add or Update Assessment template. If you update or select the new values in the Documentation and/or Findings panel, the Auto Billing button becomes available for selection. Procedure Billing After the required billing data is available in the grid, click the Verify Charges button. The Charges verified check box appears. This check box is selected and cannot be cleared. Procedure Billing If a diagnosis code is not associated with the procedure code in the Procedure Documentation Config practice template, then an alert message appears to add a diagnosis code after you click the Verify Charges button. Update the Assessment Details in Today's Assessments grid

If you want to update the assessment details in the Today's Assessments grid, Highlight an assessment in the Today's Assessments grid. Select or enter appropriate value in the Status, Side, Site, Impression, Differential Dx fields. Click Update. If the diagnosis is chronic, select the Mark diagnosis as chronic check box. If you want to add the selected assessment to the problem list, tracked problems, and/or favorite list, select the Clinical problems, My tracked problems, and/or My favorites check box respectively. Note: You can click the other tabs such as My Plan, A/P Details, and so on to document treatment plan, prescribe medications, order labs or X- rays, and request referrals.
