Upload the Procedure Diagnosis in Today's Assessment Grid

In the Documentation and/or Findings panel, select the values that are associated with the procedure diagnosis. Expand the Assessment/Plan panel and click the Auto Dx button. The Add or Update Assessment template open. The system uploads the procedure diagnosis in the Today Assessment grid. Add or Update Assessment Update the assessment details in the Today's Assessments grid. Click Save & Close. The diagnosis details appear in the grid. Update an Assessment in Today's Assessment Update an Assessment in Today's Assessment You can update the patient's assessments in the Today's Assessments grid. You can also add an assessment to the patient's clinical problems list as a chronic condition or add it to the provider's list of tracked problems and My Favorites so that it can easily be found and selected for other patients. Highlight the row with the assessment in the Today's Assessments grid. The fields in the Impression section fill with the selected assessment details. Document an Assessment

To add or edit a diagnosis on the Add or Update Assessment template, use one of the following sections: Billing Diagnosis History Clinical Problems My Favorites The new or revised diagnosis appears in the list of diagnoses in the Today's Assessment grid. If a working assessment does not exist, the diagnosis information appears in the ICD description and ICD Code fields. If your practice required a status when you selected the diagnosis, it appears in the Status field. If a working assessment exists, it remains selected in the Today's Assessment grid, and the ICD description, ICD code, and status for the working assessment are retained. To change the working assessment, select the diagnosis being addressed for this encounter in the Today's Assessment grid. The selected diagnosis becomes the new working assessment that carries forward to the other templates that share the assessment. Add Diagnostic Details

In the Assessment/Diagnosis panel, confirm the assessment selected displays in the Description details and ICD Code field. If not, select the assessment in the grid to fill in the details. Add Status, Severity, Differential Dx, and Impression. To enter specifiers in the Specifiers field, do one of the following: Select in the field and enter free text. Select the My Phrases link and select a phrase from the list. If applicable, select Present on Admission and then select a description from the list. To indicate the diagnosis is chronic, add the diagnosis to the client’s clinical problems list, and/or add the assessment to my tracked problems, select the respective checkbox. Select Add To Grid. Using the Assessments Tab

The Assessments tab in the My Plan template set houses the Add or Update Assessment template. The grids on the Assessments tab can be filtered. For example, in the Billing Diagnosis History section, selecting the Show Chronic only check box displays only those diagnoses that are not acute. Selecting the option to Add Assessments on 1 click speeds up the documentation process by including the diagnosis description and code for Today's Assessment in the grid and Problems module without having to click Add/Update. The Medications or Immunizations links can be used at any time during the encounter to access the associated module. Add New Assessments Using the Diagnosis Code Lookup Link

If the assessments that you want to add are not in the Billing Diagnosis History grid, you can add them from the Diagnosis Code Lookup link below the grid. Select the Diagnosis Code Lookup link. The diagnosis search window opens. Do one or both of the following, as needed. Enter any string of characters that may be in the clinical description for the diagnosis, and click Search. Scroll to find the diagnosis in the right-hand list. Highlight the diagnosis. Select Select. The Diagnosis search window closes and the diagnosis you selected appears in the Billing Diagnosis History grid. Select Add/Update to add the diagnosis to Today’s Assessments. When all assessments are in the Today’s Assessments grid, click Save & Close. All assessments in the Today’s Assessments grid on the Add or Update Assessment template are entered into the Today’s Assessments grid on the MICS Med/Admin OPH template.
