Evaluation Panel

The Evaluation panel on the PT Finalize template is where you can make changes to the evaluation visit type and its associated code, add up to three modifiers, enter a number of billing units, and select a diagnosis to be associated with the evaluation visit. Assessments Panel

Encounter Diagnoses and Treating Assessments can be reviewed and updated on the Assessments panel on the PT Finalize template. The Assessments panel functions the same way as the Diagnosis panel on the Home Page and the Treatment Log templates. The Encounter Diagnoses grid displays current encounter diagnoses. The Treating Assessment grid may contain the same diagnoses as the Encounter Diagnoses grid or they may be different. Add an Encounter Diagnosis

On the Subjective template, go to the General panel and click Add under the Encounter Diagnoses section. Click the Add common assessment link to view the common assessments for Physical Therapy, and do one of the following actions: If the required assessment is available on the Common Assessment window, click the assessment link, select an appropriate description from the resulting list, click OK, and then click Save & Close. If the required assessment is not available on the Common Assessment window, click the Diagnosis Code Lookup link, search for and select a diagnosis, and then click Select. Complete Billing for Procedure Items

In the Assessment panel on the PT Finalize template, select an assessment listed in the Encounter Diagnosis grid. To associate the diagnosis with a treatment, click the first available Dx code field in the row displaying the treatment. All Procedure rows displaying treatments that were performed automatically display the selected encounter diagnosis code under the Dx code field. This is done to eliminate the need to keep re-selecting the assessment and clicking in the Dx code field for the following treatment. Up to four diagnosis codes can be associated with a treatment procedure. If appropriate, click the treatment procedure’s first available Modifier field and select the modifier from the picklist. Up to three modifiers can be entered. To enter a modifier code that is not listed, select the top blank line and type the desired value. To suppress billing for a treatment procedure, select the corresponding Suppress billing check box. Click the links in the Procedures panel to document

Document Wound Care Billing Information

The Wound Care panel on the PT Finalize template is where you document billing information for wound and/or incision care performed along with other service items for testing and supplies. The Other Testing section enables you do document billing information for items such as conduction testing, range of motion testing, biofeedback training and prosthetic training. The Supplies section enables you to document billing for the use of rehabilitative durable equipment such as air pressurized beds, specialized wound dressings and specialized hospital beds. Wound Care Open the PT Finalize template and navigate to the Wound Care panel. Click the appropriate field in the desired section and select from the Service Item Master list that appears. The service item description and its CPT code appear in the corresponding fields. Click the first available Modifier field and select the modifier from the picklist. Search for a Diagnosis Code on the Common Assessments Template

If you do not see the diagnosis you need on the Common Assessments template, you can search for it. Select the Search field, and then enter the code or a word or words that describe the condition. Select Search. A search window opens with all codes related to your entry. Inf Common Assessment template Select the diagnosis code, and then select OK to add it to the Treating grid on the Common Assessments template. Select Save & Close.
