Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links allows you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document. You need to select the Include all confidential information in the social history document with patient permission checkbox on the templates to include

Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. The Social History Review section allows you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History checkbox in the Social History section of the Practice Configuration practice template to set the template as default. PRAPARE® Assessment PRAPARE® Assessment You can document information related to a patient’s social determinants of health from the Protocol for Responding to and Assessing Patient's Assets, Risks, and Experiences (PRAPARE®) form on the PRAPARE® Assessment Tool template. Providers and health centers can analyze and act on the collected data as part of the national effort to better understand the socio-economic drivers that lead to poor health outcomes and increased clinical care cost for the patients. The PRAPARE® form presents questions specific to the following areas: Personal characteristics Family and home Money and resources Social and emotional health Practice defined information Based on a patient’s response to the questions, the system calculates and displays the score and interpretation (risk level) associated with the social determinants of health data of the patient. Add Social History

Before you begin You can configure the show confidential history, panel version of adult and pediatric social history templates from the Social History section of the Practice Configuration template. For more information about these settings, go to NextGen Healthcare Success Community and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine You can add social history information in the History Summary section. The Social section uses age-specific templates based on the patient age and the Pediatric Age configuration on the Practice Configuration practice template. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Social. Select Select Social Hx to Add/Edit and select the relevant option from the list.Based on the patient age, the relevant social history template opens. Document the reported history information. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details.
