Document the Social Determinants of Health Information

You can document the social determinants of health information of a patient. Open NextGen® Enterprise EHR. On the Intake template, select any specialty and Visit type as Office Visit. To open the Social Determinants of Health template, do one of the following: In the Histories template, on the Social panel, select Social Determinants of Health. On the History Summary panel on Intake, go to Social, and select Social Determinants of Health. The Social Determinants of Health template opens. Social Determinants of Health Document the patient's response to each question. If the patient does not want to answer the questions in a specific section, select the Declines to specify check box for that section. Select Save to Grid or OK to upload the sections that calculate a score to the Screening Tool grid. Note: The applicable sections are displayed in the grid when you select OK. If the patient changes a response to a specific question, simply document the updated response and select the Save t

Patient's Social Determinants of Health

The Social Determinants of Health template enables the documentation of social, behavioral, and psychological data for the patient. The Social Determinants of Health template presents coded questions and answers specific to the following areas: Financial resource strain Education Stress Depression Physical activity Alcohol use Social connection and isolation Exposure to violence The calculated score is uploaded to the Screening Tool grid on the Social Determinants of Health template after you select Save to Grid. Note: Sections that do not calculate a score are not uploaded to the Screening Tool grid. Documenting a Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. You can view and enter confidential details provided by the patient during the course of the encounter using The Confidential link. Enter Historical Diet Details

You can document the diet details of the patient. The Social History – Diet template enables you to document specific diet details for breakfast, lunch, dinner, and snack consumed by a patient. The details on the patient's daily diet helps the clinician or provider to verify if a child gets adequate nutrition for normal growth and development. Open NextGen® Enterprise EHR. To open the Social History – Diet template, do one of the following: On the Histories template, expand the Social panel, select Diet History, and then click Add/Edit. In the History Summary panel on the Intake template, click Social, click Diet History, and then click Add/Edit. Social History – Diet Enter the date of the diet. In the Breakfast , Lunch, and Dinner section, do the following: Select the appropriate diet. Select the serving size and measure of the diet. Document Information with Other Screening Tools

You can document information using other screening tools that are not configured to open as websites or templates using the Screening Tools template. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template. For example, Intake. Select Navigation, and then select Screening Tools. The Screening Tools template opens. To view the screening tools, do one of the following: To view the screening tools configured for the selected specialty, select Current Specialty. To view all the screening tools, select All. Note: When you do not select a specialty for an encounter, the All option is selected by default, and all the screening tools appear on theScreening Tools template. Select a screening tool that is not configured to open as a website or template. Enter the following information: Score Severity or interpretation Comments Major depressive disorder pre-treatment classification Select Add. Document Diagnostic Study Results

You can document the diagnostics study results on the Diagnostic History Entry template. Open NextGen® Enterprise EHR. To open the Diagnostic History Entry template, do one of the following. On the Histories template, expand the Diagnostic Studies panel, and select Add. In the History Summary panel on the Intake template, select Diagnostic, and select Add/Edit. Select a diagnostic study from the grid. In the Study Result section, select Interpretation and select a status. In Result, enter a brief synopsis or interpretation of the result. In Comments, enter your comments or use the My Phrases link. Select Add to HPI Grid. The selected diagnostic study is added to the Reason for Visit panel as Studies Reviewed. Note: The Add to HPI Grid button is enabled only if the selected study is created in the current encounter and has a documented Result or Interpretation. Select OK.
