Document the Social Determinants of Health Information

You can capture and track a patient’s social determinants of health to better understand the non-medical factors affecting their care and outcomes. Open NextGen® Enterprise EHR. On the Intake template, select any specialty and visit type. To open the Social Determinants of Health template, do one of the following: On the Histories template, expand the Social panel, and select Social Determinants of Health. In the History Summary panel on the Intake template, select Social, and select Social Determinants of Health. Enter the patient's response to each question. Note: To configure the questions and their answer options, go to NextGen Healthcare Success Community, and download the Coded Results Mapping User Guide for NextGen® Adaptive Content Engine. If the patient does not want to answer the questions in any of the sections, select the corresponding Declines to specify checkbox. Select Save to Grid or OK. Patient's Social Determinants of Health

The Social Determinants of Health template enables the documentation of social, behavioral, and psychological data for the patient. The Social Determinants of Health template presents coded questions and answers specific to the following areas: Financial resource strain Education Stress Depression Physical activity Alcohol use Social connection and isolation Exposure to violence The calculated score is uploaded to the Screening Tool grid on the Social Determinants of Health template after you select Save to Grid. Note: Sections that do not calculate a score are not uploaded to the Screening Tool grid. Enter Historical Diet Details

You can document the diet details of the patient. The Social History – Diet template enables you to document specific diet details for breakfast, lunch, dinner, and snack consumed by a patient. The details on the patient's daily diet helps the clinician or provider to verify if a child gets adequate nutrition for normal growth and development. Open NextGen® Enterprise EHR. To open the Social History – Diet template, do one of the following: On the Histories template, expand the Social panel, select Diet History, and then click Add/Edit. In the History Summary panel on the Intake template, click Social, click Diet History, and then click Add/Edit. Social History – Diet Enter the date of the diet. In the Breakfast , Lunch, and Dinner section, do the following: Select the appropriate diet. Select the serving size and measure of the diet. Document Diagnostic Study Results

You can document the diagnostics study results on the Diagnostic History Entry template. Open NextGen® Enterprise EHR. To open the Diagnostic History Entry template, do one of the following. On the Histories template, expand the Diagnostic Studies panel, and select Add. In the History Summary panel on the Intake template, select Diagnostic, and select Add/Edit. Select a diagnostic study from the grid. In the Study Result section, select Interpretation and select a status. In Result, enter a brief synopsis or interpretation of the result. In Comments, enter your comments or use the My Phrases link. Select Add to HPI Grid. The selected diagnostic study is added to the Reason for Visit panel as Studies Reviewed. Note: The Add to HPI Grid button is enabled only if the selected study is created in the current encounter and has a documented Result or Interpretation. Select OK. Document the Patient's Social Support Information

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. From the list, select Comp Assess - Social Hx, and then select OK. In the Living Situation panel, document the patient's current living details such as address, residential treatment, household composition, and risk assessment. In the Family Information panel, document details about the patient's family members, family history of mental illness or substance addiction, and the emotions attached to the family members. In the Development panel, document the patient's motor and sensory development history. In the Abuse/Sexual Risk Behavior panel, document the details about any physical, sexual, or emotional abuse the patient has experienced. You can document the diagnostics study results in the Diagnostic History Entry template. Open NextGen® Enterprise EHR. Open the Histories template. In the Diagnostic Studies panel, select Add.The Diagnostic History Entry template opens. Select a diagnostic study from the grid. In the Study Result section, select Interpretation and select a status. In Result, enter a brief synopsis or interpretation of the result. In Comments, enter your comments or use the My Phrases link.
