Patient's Social Determinants of Health

The Social Determinants of Health template enables the documentation of social, behavioral, and psychological data for the patient. The Social Determinants of Health template presents coded questions and answers specific to the following areas: Financial resource strain Education Stress Depression Physical activity Alcohol use Social connection and isolation Exposure to violence The calculated score is uploaded to the Screening Tool grid on the Social Determinants of Health template after you select Save to Grid. Note: Sections that do not calculate a score are not uploaded to the Screening Tool grid. Document a Patient’s Social Determinants on the PRAPARE® Form

You can document a patient's social determinants in the PRAPARE® assessment tool. If the patient declines to perform the assessment, you can document that as well. When you select the Decline PRAPARE® checkbox, all documented details, except the automatically filled information, will be cleared. If you continue to document details after declining, the Decline PRAPARE® checkbox will be cleared. Open the PRAPARE® Assessment Tool template. To change the date, select in the Performed Date field.By default, the encounter date appears in this field. You can only enter the current or a past encounter date. To view the PRAPARE® details entered in the previous encounters, select the PRAPARE® Flowsheet link. To edit the patient's personal information, select Personal Information.By default, the patient's basic demographic details are filled from the Modify Patient Information template. Submit the PRAPARE® Assessment for Review

When a patient's PRAPARE® assessment needs to be reviewed, you can submit the PRAPARE® form to a recipient within the same practice as a task. Open the PRAPARE® Assessment Tool template. Document the patient’s social determinants. In the Enabling services provided section, do one of the following: To include services for review, select Yes, select a service, and then select . To exclude all services from review, select No. Note: The list of available services is configurable using the Enabling Services custom DB picklist. For more information on how to customize the DB picklist, go to NextGen Healthcare Success Community, and download the Custom DB Picklists System Template for NextGen® Adaptive Content Engine. Prerequisites for PRAPARE® Assessment

You need to configure the PRAPARE® Configuration template to manage score related to each response documented on the PRAPARE® form, set up the risk level associated with the Total Score, and set the pediatric age. To calculate the Total Score and Interpretation on the PRAPARE® form, you need to configure: Scoring - the system is packaged with preconfigured values provided by the National Association of Community Health Centers (NACHC) in the PRAPARE® Scoring Configuration panel on the PRAPARE® Configuration template. You can edit the existing values. Based on the configuration, a score is assigned to the response captured in the PRAPARE® form. Interpretation (risk level) - to set up the risk level associated with the Total Score that appears on the PRAPARE® form, use the PRAPARE® Total Score and Interpretation Configuration panel on the PRAPARE® Configuration template. To set the pediatric age limit, you need to configure the Peds age for PRAPARE® on the PRAPARE® Configuration template

Open the PRAPARE Assessment Tool

You can open the PRAPARE® assessment tool from multiple locations. Before you begin To enable the PRAPARE® link in the Social panel of the Histories template and the History Summary panel of the Intake and SOAP templates, you must configure the PRAPARE® in the Practice Configuration template. Open NextGen® Enterprise EHR. Open a patient record and create an encounter. To open the PRAPARE® Assessment Tool template, do one of the following: Open the Intake template, expand the History Summary panel, and then select PRAPARE®. Open the Histories template, expand the Social panel, and then select PRAPARE®. On the SOAP template, expand the History panel, and then select PRAPARE®. In the Encounter History tab, select Open Template to open the Select Template window. Select the Medical Records tab. Under the Show section, select All. Generate the PRAPARE® Document

You can follow the steps to generate the prepare document. Open NextGen® Enterprise EHR. Open the PRAPARE® Assessment Tool template. Document your assessment of the patient in each section of the PRAPARE form. To generate the document, on Top Toolbar, select Documents.
